Neutralization against Omicron variant in transplant recipients after three doses of mRNA vaccine

Deepali Kumar, Queenie Hu, Reuben Samson, Victor H. Ferreira, Victoria G. Hall, Matthew Ierullo, Beata Majchrzak-Kita, William Hardy, Anne-Claude Gingras, Atul Humar

Summary: The Omicron variant of the SARS-CoV-2 virus has become the dominant circulating strain globally, and three doses of mRNA COVID-19 vaccine are recommended for transplant recipients. However, there is limited research on the immunogenicity of mRNA vaccines specifically against the Omicron variant. This study analyzed the neutralization effects of Omicron in transplant recipients who received three doses of mRNA-1273 vaccine, and the results showed poor neutralizing responses against the Omicron variant in these patients.

Surgical Management of Gastric Cancer A Review

George Z. Li, Gerard M. Doherty, Jiping Wang

Summary: Surgery is crucial in the management of gastric cancer at all stages. Early gastric cancer patients with low risk of lymph node metastasis can potentially be cured with endoscopic therapy or surgery alone. Patients with later-stage localized gastric cancer benefit from extensive lymphadenectomy and multimodality therapy. Ongoing studies are investigating novel surgical approaches and peritoneal-directed therapies.

Survival After Hyperthermic Intraperitoneal Chemotherapy and Primary or Interval Cytoreductive Surgery in Ovarian Cancer A Randomized Clinical Trial

Myong Cheol Lim, Suk-Joon Chang, Boram Park, Heon Jong Yoo, Chong Woo Yoo, Byung Ho Nam, Sang-Yoon Park

Summary: The study aimed to evaluate the clinical benefit of HIPEC in patients with ovarian cancer. The results showed that adding HIPEC to cytoreductive surgery did not improve survival rates for patients with advanced ovarian cancer. However, in the subgroup of interval cytoreductive surgery, the addition of HIPEC improved progression-free and overall survival.

The Most Current Algorithms for the Treatment and Prevention of Hypertrophic Scars and Keloids: A 2020 Update of the Algorithms Published 10 Years Ago

Rei Ogawa

Summary: This article reviewed and updated the literature on hypertrophic scars and keloids. The findings suggest that treatment methods depend on the severity and number of scars, while prevention methods include local factors, systemic factors, genetic factors, and lifestyle factors. Conservative therapy is the preferred treatment for keloids, but surgery is necessary for severe scars.

Early clinical experience with nirmatrelvir/ritonavir for the treatment of COVID-19 in solid organ transplant recipients

David M. Salerno, Douglas L. Jennings, Nicholas W. Lange, Danielle (Bley) Kovac, Tara Shertel, Justin K. Chen, Jessica Hedvat, Jenna Scheffert, Robert S. Brown, Marcus R. Pereira

Summary: The use of Nirmatrelvir/ritonavir (NR) in solid organ transplant recipients (SOTRs) with mild COVID-19 has not been described yet. The objective of this study was to evaluate outcomes among SOTRs and describe the drug-drug interaction of NR. The results show that the clinically significant interaction between NR and immunosuppressive agents can be reasonably managed with a standardized dosing protocol.

Effect of Laparoscopic Sleeve Gastrectomy vs Roux-en-Y Gastric Bypass on Weight Loss, Comorbidities, and Reflux at 10 Years in Adult Patients With Obesity The SLEEVEPASS Randomized Clinical Trial

Paulina Salminen, Sofia Gronroos, Mika Helmio, Saija Hurme, Anne Juuti, Risto Juusela, Pipsa Peromaa-Haavisto, Marja Leivonen, Pirjo Nuutila, Jari Ovaska

Summary: This study compared the long-term outcomes of laparoscopic sleeve gastrectomy (LSG) and laparoscopic Roux-en-Y gastric bypass (LRYGB) at 10 years. The results showed that LRYGB resulted in greater weight loss compared to LSG. However, both procedures were effective in achieving sustainable weight loss. LSG was associated with a higher prevalence of gastroesophageal reflux symptoms, while the occurrence of Barrett esophagus was similar between the two procedures.

European Stroke Organisation (ESO)-European Society for Minimally Invasive Neurological Therapy (ESMINT) expedited recommendation on indication for intravenous thrombolysis before mechanical thrombectomy in patients with acute ischemic stroke and anterior circulation large vessel occlusion

Schellinger, Daniel Strbian, Danilo Toni, Philip White, William Whiteley, Andrea Zini, Wim van Zwam, Jens Fiehler

Summary: This study evaluated whether mechanical thrombectomy (MT) alone is non-inferior to intravenous thrombolysis (IVT) plus MT in patients with anterior circulation large vessel occlusion (LVO) ischemic stroke. The recommendations suggest using IVT plus MT for patients directly admitted to a MT-capable center within 4.5 hours of symptoms onset, and IVT followed by rapid transfer to a MT-capable center for patients admitted to a center without MT facilities.

Yutian Zou, Jindong Xie, Shaoquan Zheng, Wenqing Liu, Yuhui Tang, Wenwen Tian, Xinpei Deng, Linyu Wu, Yue Zhang, Duxun Tan, Qing Liu, Xiaoming Xie

Summary: A cell death index (CDI) model was established to accurately predict the clinical prognosis and drug sensitivity of triple-negative breast cancer (TNBC) after surgery. Patients with high CDI had a worse prognosis and were associated with immune checkpoint genes and tumor microenvironment components.

Our contemporary understanding of the aetiology of pressure ulcers/pressure injuries

Amit Gefen, David M. Brienza, Janet Cuddigan, Emily Haesler, Jan Kottner

Summary: The third edition of the Clinical Practice Guideline on Prevention and Treatment of Pressure Ulcers/Injuries, published in 2019, provides clinicians with updated evidence-based guidance on pressure ulcers. The guideline also discusses the etiology, classification, and future research needs related to pressure ulcers. Research shows that sustained soft tissue deformations can lead to cell death, tissue damage, and the formation of pressure ulcers/pressure injuries. In order to prevent pressure ulcers, it is important to minimize deformations in tissues by reducing peak strain/stress values or decreasing exposure time.

Summary: This study compared the outcomes of robot-assisted minimally invasive esophagectomy (RAMIE) and conventional minimally invasive esophagectomy (MIE) in the treatment of esophageal squamous cell carcinoma (ESCC). The results showed that RAMIE had shorter operation time and better lymph node dissection in patients who received neoadjuvant therapy. RAMIE and MIE had similar rates of complications and postoperative complications.
