Steele, Michel Suter, Shanu N. Kothari

Summary: Major updates to the 1991 National Institutes of Health guidelines for bariatric surgery recommend metabolic and bariatric surgery (MBS) for individuals with a body mass index (BMI) >35 kg/m2 regardless of co-morbidities. It is also suggested that individuals with metabolic disease and a BMI of 30-34.9 kg/m2 should consider MBS. Furthermore, BMI thresholds should be adjusted for the Asian population, with a BMI >25 kg/m2 indicating clinical obesity, and MBS offered to individuals with a BMI >27.5 kg/m2. The long-term safety and efficacy of MBS have been consistently demonstrated, and selected children and adolescents may also be considered for MBS.

MRI-Diagnosed Tumor Deposits and EMVI Status Have Superior Prognostic Accuracy to Current Clinical TNM Staging in Rectal Cancer

Amy C. Lord, Nigel D'Souza, Annabel Shaw, Zena Rokan, Brendan Moran, Muti Abulafi, Shahnawaz Rasheed, Anuradha Chandramohan, Alison Corr, Ian Chau, Gina Brown

Summary: The presence of nonnodal tumor nodules (mrTDs) and lymph node metastases (mrLNMs) in rectal cancer can predict different prognostic outcomes. Patients with mrTDs and mrEMVI have poorer survival and are more likely to experience distant recurrence. Compared to traditional T and N staging, mrTDs and mrEMVI have higher prognostic accuracy and can be used to determine treatment and follow-up protocols.

GLOBOCAN 2020 Report on Global Cancer Burden: Challenges and Opportunities for Surgical Oncologists

S. V. S. Deo, Jyoti Sharma, Sunil Kumar

Summary: Cancer is becoming a major global public health challenge, with the burden of cancer increasing globally according to the GLOBOCAN 2020 report which estimates the occurrence of 36 cancers in 185 countries. There are global disparities in cancer incidence and mortality in different geographic regions, and the human development index (HDI) also affects cancer incidence and projected global burden. Surgical oncologists play a crucial role in future global cancer control strategies.

The Risk of Postoperative Complications After Major Elective Surgery in Active or Resolved COVID-19 in the United States

John Z. Deng, Janine S. Chan, Alexandra L. Potter, Ya-Wen Chen, Harpal S. Sandhu, Nikhil Panda, David C. Chang, Chi-Fu Jeffrey Yang

Summary: Surgical timing relative to the development of Covid-19 is associated with the risks of postoperative complications. Patients who undergo major elective surgery within 0 to 4 weeks after Covid-19 infection have an increased risk of postoperative pneumonia, respiratory failure, pulmonary embolism, and sepsis. Surgery performed within 4 to 8 weeks after infection is still associated with an increased risk of postoperative pneumonia, but surgery 8 weeks after Covid-19 diagnosis is not associated with increased complications.
