Post-Stroke Cognitive Impairment and Dementia

Natalia S. Rost, Amy Brodtmann, Matthew P. Pase, Susanne J. van Veluw, Alessandro Biffi, Marco Duering, Jason D. Hinman, Martin Dichgans

Summary: Poststroke cognitive impairment and dementia is a common problem among stroke patients, influenced by factors such as age, education level, preexisting cognitive decline, and history of prior stroke. Manifestations of cerebral small vessel disease also contribute to cognitive outcomes. Understanding the interaction between acute stroke events and preexisting brain pathology is crucial for personalized prediction, prevention, targeted interventions, and rehabilitation.

Bradbury, Lisa Broxmeyer, Jean Marie Connors, Anna Falanga, Toshiaki Iba, Scott Kaatz, Jerrold H. Levy, Saskia Middeldorp, Tracy Minichiello, Eduardo Ramacciotti, Charles Marc Samama, Jecko Thachil

Summary: Antithrombotic agents, particularly heparins, are recommended for patients with COVID-19 to reduce thromboembolism risk. The optimal dosing and timing of these treatments, as well as the benefits of other antithrombotic agents, are still unclear. A panel of experts developed 12 recommendations based on high- to moderate-quality evidence, including the use of prophylactic or therapeutic dose heparin for different patient groups. Additional weak recommendations were made for the use of sulodexide, adding antiplatelet agents, and prophylactic rivaroxaban in select patients. Future updates will incorporate new evidence and may modify these recommendations.

Niteesh K. Choudhry, Ian M. Kronish, Wanpen Vongpatanasin, Keith C. Ferdinand, Valory N. Pavlik, Brent M. Egan, Antoinette Schoenthaler, Nancy Houston Miller, David J. Hyman

Summary: The widespread treatment of hypertension and improvements in blood pressure have led to a decline in heart disease and stroke, but there is still a gap between public health targets and actual blood pressure control rates. Medication nonadherence is a key factor contributing to this gap, and the scientific statement aims to summarize its impact and strategies for improving adherence.

Takotsubo Syndrome: Pathophysiology, Emerging Concepts, and Clinical Implications

Trisha Singh, Hilal Khan, David T. Gamble, Caroline Scally, David E. Newby, Dana Dawson

Summary: “Takotsubo syndrome is an acute condition with transient dysfunction of the left ventricle, which is challenging to differentiate from acute myocardial infarction. Recent studies have shown that patients with this syndrome have persistent cardiac dysfunction and limiting symptoms. They also have a substantial burden of morbidity and mortality, similar to patients with acute coronary syndrome. The underlying pathophysiology of this condition remains unclear, and further research is needed to inform evidence-based management strategies.”

Summary: Incidence of vascular diseases remains elevated up to 49 weeks after COVID-19 diagnosis, with a faster decline for arterial thromboses than VTEs. Patients hospitalized with COVID-19, non-White individuals, and those without previous events are at higher risk. Policies to prevent severe COVID-19, including vaccination, early post-discharge review, risk factor control, and secondary preventive agents, are supported by these findings.
