Pregnant and breastfeeding women's attitudes and fears regarding the COVID-19 vaccination

Nora K. Schaal, Janine Zoellkau, Philip Hepp, Tanja Fehm, Carsten Hagenbeck

Summary: The survey revealed that during pregnancy, the majority of women (57.4%) are not in favor of receiving the COVID-19 vaccine, while breastfeeding women show higher willingness (39.5% are in favor). The willingness to be vaccinated is significantly related to the women's anxiety levels of getting infected and developing disease symptoms, with main reasons for vaccination hesitancy including limited vaccine-specific information and scientific evidence on vaccine safety, as well as fear of harming the fetus or infant.

Fetal exome sequencing for isolated increased nuchal translucency: should we be doing it?

R. Mellis, R. Y. Eberhardt, S. J. Hamilton, D. J. McMullan, M. D. Kilby, E. R. Maher, M. E. Hurles, J. L. Giordano, V Aggarwal, D. B. Goldstein, R. J. Wapner, L. S. Chitty

Summary: Prenatal exome sequencing has a low diagnostic yield for fetuses with isolated increased nuchal translucency, but significantly higher when there are additional structural anomalies present. It may not be appropriate for truly isolated increased NT cases, but careful ultrasound scanning to identify other anomalies emerging later can direct testing to focus where there is a higher likelihood of diagnosis.

Laura A. Magee, Graeme N. Smith, Christine Bloch, Anne-Marie Cote, Venu Jain, Kara Nerenberg, Peter von Dadelszen, Michael Helewa, Evelyne Rey

Summary: This guideline reviews the diagnosis, evaluation, and management of hypertensive disorders of pregnancy (HDPs), prediction and prevention of preeclampsia, and postpartum care. The recommendations are based on a comprehensive literature review and prioritize randomized controlled trials and systematic reviews to support therapeutic recommendations. Spectrum of neonatal COVID-19 in Iran: 19 infants with SARS-CoV-2 perinatal infections with varying test results, clinical findings and outcomes

David A. Schwartz, Parisa Mohagheghi, Babak Beigi, Nazanin Zafaranloo, Fereshteh Moshfegh, Anita Yazdani

Summary: This study analyzed the epidemiology, diagnostic test results, and clinical findings of neonatal COVID-19 in Iran. The results showed significant variation in the cases, with some neonates initially testing negative but later becoming positive. Two neonates were highly suspicious for intrauterine vertical transmission. The study recommends repeat testing for neonates testing negative initially to avoid misdiagnosis.

Management and implications of severe COVID-19 in pregnancy in the UK: data from the UK Obstetric Surveillance System national cohort

Nicola Vousden, Rema Ramakrishnan, Kathryn Bunch, Edward Morris, Nigel Simpson, Christopher Gale, Pat O'Brien, Maria Quigley, Peter Brocklehurst, Jennifer J. Kurinczuk, Marian Knight

Summary: Severe COVID-19 in pregnancy increases the risk of adverse outcomes. Factors such as age, overweight or obesity, mixed ethnicity, and gestational diabetes are associated with severe infection. Cesarean birth, preterm birth, stillbirth, and neonatal unit admission are more likely to occur in women with severe infection. Most women with severe infection who gave birth at a later admission delivered at 36 weeks or later, but rapid delivery may be warranted for those with severe infection at term.

Progress in the understanding of the pathophysiology of immunologic maladaptation related to early-onset preeclampsia and metabolic syndrome related to late-onset preeclampsia

Pierre-Yves Robillard, Gustaaf Dekker, Marco Scioscia, Shigeru Saito

Summary: Human reproduction has two notable features: deep endovascular trophoblast invasion and preeclampsia. Regulatory T cells and sexual intercourse play important roles in maintaining pregnancy. Preeclampsia is associated with immune and cardiovascular metabolic factors. Preventative measures for preeclampsia include low-dose aspirin and controlling weight gain.

The effects of the COVID 19 pandemic on vaccine decisions in pregnant women

Hatice Gencer, Sevgi Ozkan, Okan Vardar, Pinar Serceku

Summary: This study aims to understand the opinions of pregnant women on vaccinations during pregnancy and childhood, and the impact of the COVID-19 pandemic on these views. The results show that approximately 29.6% of pregnant women exhibited hesitant attitudes towards vaccinations when using forum websites. Vaccine hesitancy was found to be higher among pregnant women with lower economic levels and concerns about vaccination risks. The COVID-19 pandemic was reported to have decreased vaccine hesitancy in 28.9% of the participants.

COVID-19 and maternal, fetal and neonatal mortality: a systematic review

Kamran Hessami, Nahid Homayoon, Atefe Hashemi, Homeira Vafaei, Maryam Kasraeian, Nasrin Asadi

Summary: This comprehensive review examines the available evidence on maternal, fetal, and neonatal mortality associated with COVID-19 infection. The results suggest that maternal mortality is more common in women with pre-existing conditions, while fetal and neonatal mortality may be a result of maternal infection severity or prematurity. However, further research is needed to determine the exact magnitude of these mortality rates.

Proteinuria during pregnancy: definition, pathophysiology, methodology, and clinical significance

Michal Fishel Bartal, Marshall D. Lindheimer, Baha M. Sibai

Summary: In pregnancy, the measurement of urine protein excretion is common, and recent guidelines suggest that proteinuria alone is sufficient for the diagnosis of preeclampsia. While a 24-hour urine collection is the reference standard, the urine protein-to-creatinine ratio is a reasonable test to rule out proteinuria.

Gut Microbiota in Patients with Polycystic Ovary Syndrome: a Systematic Review

Jingbo Guo, Jie Shao, Yuan Yang, Xiaodan Niu, Juan Liao, Qing Zhao, Donghui Wang, Shuaitong Li, Junping Hu

Summary: The relationship between gut microbiome and Polycystic Ovary Syndrome (PCOS) remains unclear, with existing studies showing differences in specific gut microbes between patients with PCOS and healthy controls. PCOS patients exhibit reduced alpha-diversity and significant differences in beta-diversity composition compared to healthy controls, with common bacterial alterations identified. More research is needed to determine the role of these microbes in the development of PCOS.
