Safety and efficacy of dexmedetomidine vs ketamine vs midazolam combined with propofol in gastrointestinal endoscopy for cancer patients: A randomized double-blinded trial

Summary: This study aimed to compare the effects of different drugs, including dexmedetomidine, ketamine, and midazolam, in combination with propofol for sedation in cancer patients undergoing GI endoscopy. The results showed that dexmedetomidine-propofol and ketamine-propofol had better sedation efficacy compared to midazolam-propofol, with the superior sedative effect of ketamine-propofol than dexmedetomidine-propofol. Ketamine-propofol also had more stable heart rate and mean arterial pressure.

Intraperitoneal Versus Intravenous Dexmedetomidine for Postoperative Analgesia Following Laparoscopic Sleeve Gastrectomy Surgery: A prospective, Randomized Controlled trial

Summary: The study aimed to evaluate the efficacy of dexmedetomidine in combination with bupivacaine for postoperative pain control after laparoscopic sleeve gastrectomy. The results showed that intraperitoneal dexmedetomidine prolonged the duration of analgesia and reduced the consumption of analgesics compared to intravenous dexmedetomidine. There was a significant difference in visual analog scale scores at 6, 12, and 18 hours postoperatively.

Effects of high-dose L-carnitine supplementation on diaphragmatic function in patients with respiratory failure: A randomized clinical trial

Summary: The study aimed to assess the impact of high and low-dose L-carnitine supplementation on diaphragmatic muscle function. The results showed that high-dose L-carnitine had a positive effect on diaphragmatic excursion, while it had no significant effect on diaphragmatic thickening. Additionally, serum levels of L-carnitine were positively correlated with diaphragmatic function measures.

The effect of melatonin administration on sedation level as adjuvant to propofol in mechanically ventilated traumatic brain injury patients

Prevalence of musculoskeletal disorders in anesthesiologists in Ismailia Governorate

Summary: This study investigated the prevalence of work-related musculoskeletal disorders (WRMSDs) among anesthesiologists in Ismailia/Egypt, as well as their related risk factors. The results showed that a significant proportion of anesthesiologists suffer from WRMSDs, with back pain and neck pain being the most commonly reported symptoms. Possible risk factors included excessive bending and twisting, poor work ergonomics, and suboptimal patient positioning. Anesthesiologists also reported sleep difficulties, anxiety, and the need for medication to control pain, but did not require sick leave.

Ultrasound -guided erector spinae plane block (ESPB) versus intravenous opioids based analgesia in patients with rib fractures

Summary: This study compared the effectiveness of the erector spinae plane block (ESPB) with opioid-based analgesia in relieving pain in rib fractures patients. The results showed that ESPB provided superior analgesia and improved respiratory function compared to opioid-based analgesia. Additionally, ESPB was associated with fewer side effects, less opioid consumption, and better patient satisfaction.

Evaluation of Ambu-Aura-i laryngeal mask as a conduit for endotracheal intubation. A comparison with Air-Q intubating laryngeal airway in adult surgical patients

Summary: This study compared the performance of the Ambu-Aura-i laryngeal mask and the Air-Q Intubating Laryngeal airway as conduits for endotracheal intubation. The Aura-i group showed shorter insertion time, longer time to remove the device, and higher airway leak pressure compared to the Air-Q group. Both devices had similar number of insertion attempts, time to insert an endotracheal tube, and fibre-optic laryngoscopic view grades. The study concluded that both the Ambu-Aura-i laryngeal mask and the Air-Q intubating laryngeal airway are efficient conduits for endotracheal intubation, with advantages for the Ambu-Aura-i mask in terms of device insertion time and use during mechanical ventilation.

Comparison between levosimendan versus beta agonists in preservation of renal function in cardiac surgery patients with low cardiac output syndrome

Summary: This study aimed to evaluate the nephroprotective effect of levosimendan compared to beta agonists in patients with LCOS after cardiac surgery. The results showed that at discharge from ICU, the incidence of renal failure was 13.3% in the beta agonist group and 0% in the levosimendan group. Therefore, levosimendan may have a better nephroprotective effect in reducing the incidence of kidney failure in patients with LCOS after cardiac surgery. Rating: 7 out of 10.

Role of perfusion index in early detection of hypotension following spinal anesthesia for cesarean section

Summary: Spinal hypotension is a common drawback of spinal anesthesia in obstetric anesthesia. The standard method of blood pressure measurement may miss episodes of hypotension within a reasonable time frame. This study found that perfusion index (PI) can predict spinal hypotension, highlighting its importance for early detection and treatment.

Lung ultrasound compared to fiber-optic bronchoscopy in detecting proper endotracheal tube position in intubated patients in the intensive care unit

Summary: This cross-sectional study estimates lung ultrasound's sensitivity in detecting the proper position of the endotracheal tube compared to auscultation and fiberoptic bronchoscopy. The results showed that lung ultrasound has a very high sensitivity for detecting the proper endotracheal tube position, compared to fiberoptic bronchoscopy and auscultation.

Effect of terlipressin on systemic and hepatic hemodynamics in patients undergoing liver transplantation

Risk factors influence the arterial line patency in ICU-bound patients during COVID-19 pandemic: An observational cohort study

Summary: This study examined the effects of systemic anticoagulation on arterial catheter patency in COVID-19 patients. The results showed that systemic anticoagulation was associated with a lower failure rate of arterial catheters compared to non-systemic anticoagulation. Platelet level, ultrasound use during catheter insertion, and prone position in the ICU were identified as independent predictors of arterial catheter failure.

Role of adding dexmedetomidine, ketamine, and magnesium sulphate to caudal block as preemptive analgesia in hypospadias repair in pediatrics: A randomized double-blinded prospective trial

Analgesic efficacy of pre-emptive ultrasound-guided mid-point transverse process to pleura block for patients undergoing posterolateral thoracotomy incisions: Randomized controlled trial

Summary: This research aimed to assess the safety and efficacy of ultrasound-guided preemptive mid-point transverse process to pleura block for posterolateral thoracotomy. The results showed that the MTP block group had lower pain scores, lesser analgesics consumption, and reduced risk of chronic pain compared to the sham block group. Oxygen saturation and SpO2/FiO2 also significantly improved in the MTP block group. The US-guided MTP block provides effective analgesia for posterolateral thoracotomy.

Influence of pre-peritoneal vs. epidural levobupivacaine infusion on troponin I and BNP as predictors of cardiac injury in cancer patients undergoing major upper abdominal operations: A randomized controlled clinical trial

Summary: This study compared the effects of two analgesic methods in cancer patients undergoing major abdominal surgery. The results showed that preperitoneal analgesia and epidural analgesia had similar analgesic effects, but preperitoneal analgesia group showed better performance in cardiovascular side effects. Therefore, preperitoneal analgesia can be used as an alternative to epidural analgesia.

Ultrasound-guided ilioinguinal/iliohypogastric nerve block compared to posterior quadratus lumborum block in patients undergoing inguinal hernia repair

Summary: This study compared the analgesic effect of ilioinguinal/iliohypogastric nerve block (IINB) and posterior quadratus lumborum block (PQLB) in adults undergoing unilateral inguinal hernia repair. The results showed that PQLB was superior to IINB in terms of better pain control, lower heart rate and mean arterial pressure, decreased opioid consumption, and higher patient satisfaction.

A stepwise management approach to an unexpected cause of a yellow CSF tap; A case report

Summary: A yellow CSF tap could indicate contamination with Povidone-iodine. Contaminated spinal needles may cause chemical meningitis. Disposable spinal trays should be used for skin preparation to prevent CSF contamination. A stepwise management protocol is recommended to reduce the incidence of chemical meningitis.

Continuous thoracic epidural versus continuous paravertebral analgesia in patients undergoing open renal surgery: Evaluation of pulmonary function; randomized double-blinded clinical trial

Feasibility of combined ultrasound guided interscalene and erector spinae plane block for regional anesthesia in modified radical mastectomy with axillary lymph node dissection: A pilot study

Summary: The combination of erector spinae plane block (ESPB) and interscalene block (IBPB) can be used as an alternative to general anesthesia (GA) for modified radical mastectomy with axillary lymph node dissection (MRM-ALND) in high-risk patients. This technique has a high success rate, provides satisfactory postoperative analgesia, and reduces the potential risks associated with GA.
