Serial bone scans of a case of Waldenstrom macroglubinemia that show progressive involvement of the sternum, clavicles, and ribs are presented. The pattern of focally increased uptake seen on bone scans associated with the patient's clinical presentation is similar to an inflammatory process. Awareness of this pattern could help in suspecting the condition if undiagnosed or making the proper interpretation when diagnosis is known as due to infiltration by this lymphoproliferative disorder rather than due to other reasons such as infection or metabolic bone disease.
