Novel role for tumor suppressor gene, liver kinase B1, in epithelial-mesenchymal transition leading to chronic lung allograft dysfunction

Mohammad Rahman et al.

Summary: The study investigates the role of LKB1 in the development of CLAD following lung transplantation, revealing that downregulation of LKB1 is associated with EMT occurrence. Exosomes from lungs undergoing chronic rejection are linked to inactivation of LKB1, leading to EMT and the pathogenesis of CLAD.

Restrictive allograft syndrome vs bronchiolitis obliterans syndrome: Immunological and molecular characterization of circulating exosomes

Sandhya Bansal et al.

Summary: Chronic lung allograft dysfunction in lung transplant recipients has two distinct phenotypes, and exosomes from patients with restrictive allograft syndrome (RAS) have different immune and molecular characteristics compared to those with obstructive bronchiolitis obliterans syndrome (BOS). RAS exosomes contain high levels of proinflammatory cytokines, HLA class II molecules, lung self-antigens, and antibodies to HLA class II molecules, indicating severe allograft injury.

Decline in Club Cell Secretory Proteins, Exosomes Induction and Immune Responses to Lung Self-antigens, Kα1 Tubulin and Collagen V, Leading to Chronic Rejection After Human Lung Transplantation

Yoshihiro Itabashi et al.

Summary: The study found that lung transplant recipients with BOS had lower CCSP levels up to 9 months before BOS diagnosis, and those with SAgs antibodies 1 year post-transplant also developed DSA and had lower CCSP. Exosomes from BOS contained increased SAgs, NK cells markers, and cytotoxic molecules.

Cutting Edge: Circulating Exosomes with COVID Spike Protein Are Induced by BNT162b2 (Pfizer-BioNTech) Vaccination prior to Development of Antibodies: A Novel Mechanism for Immune Activation by mRNA Vaccines

Sandhya Bansal et al.

Summary: After vaccination, circulating exosomes rapidly induce expression of SARS-CoV-2 spike protein, with antibodies peaking 14 days after the second dose. Increasing levels of spike protein in exosomes following booster doses demonstrate their important role in effective immunization post mRNA vaccination.

Circulating exosomes induced by respiratory viral infections in lung transplant recipients activate cellular stress, innate immune pathways and epithelial to mesenchymal transition

Sandhya Bansal et al.

Summary: The study found that exosomes from lung transplant recipients with respiratory viral infection contain nucleic acids capable of inducing innate immune signaling, endoplasmic reticulum stress, and epithelial mesenchymal transition. Therefore, the authors propose that respiratory viral infection may lead to the generation of exosomes that initiate the process of CLAD in mouse models. The novel findings identify the molecular mechanisms through which respiratory viral infection increases the risk of CLAD.

Small Extracellular Vesicles in Transplant Rejection

Justyna E. Golebiewska et al.

Summary: Small extracellular vesicles (sEV) released by human cells have the ability to stimulate or inhibit immune responses through multiple mechanisms, carrying antigens on their surface. Studies show that sEV play a role in rejection or tolerance in solid organ and tissue allotransplantation, potentially serving as biomarkers for detecting ongoing allograft rejection.

Graft-derived extracellular vesicles transported across subcapsular sinus macrophages elicit B cell alloimmunity after transplantation

Furong Zeng et al.

Summary: It has been found that donor MHC antigens are released from skin or cardiac allografts via extracellular vesicles (EVs) and captured by macrophages in lymph nodes or spleen, triggering B cell activation and DSA production. This unexpected role of graft-derived EVs opens up possibilities to interfere with EV biogenesis, trafficking, or function to restrict alloreactive B cell priming.

A decline in club cell secretory proteins in lung transplantation is associated with release of natural killer cells exosomes leading to chronic rejection

Ranjithkumar Ravichandran et al.

Summary: The study explored the mechanisms by which a decline in CCSP contributes to chronic lung allograft rejection in mouse models. It was found that induction of exosomes led to immune responses to donor MHC and lung self-antigens, resulting in CCSP decline, NK cell infiltration, and release of exosomes from NK cells. The results suggest a novel role for exosomes derived from NK cells in the pathogenesis of chronic lung allograft rejection.

Summary: Lung transplantation is an established treatment for end-stage respiratory failure, but ischemia-reperfusion injury (IRI) remains a significant complication.
