Migrainous vertigo is an emergent concept that links recurrent vertigo causally to migraine. The usual presentation includes attacks of spontaneous or positional vertigo lasting seconds to days accompanied by migrainous symptoms or provoked by typical migraine triggers. Because headache often is absent during acute attacks, other migrainous features have to be unearthed by thorough history taking. Early diagnosis is essential to target acute and prophylactic treatment at the underlying migraine.
