Improving translational research in sex-specific effects of comorbidities and risk factors in ischaemic heart disease and cardioprotection: position paper and recommendations of the ESC Working Group on Cellular Biology of the Hear

Cinzia Perrino et al.

Summary: Ischaemic heart disease (IHD) is a major cause of death in both men and women, with sex playing an important role in various aspects of the disease. Differences in risk factors and comorbidities between males and females can impact the development and prognosis of IHD, but the cellular and molecular mechanisms underlying these differences are not well understood. Future research should focus on investigating these sex-specific differences to improve prediction and prevention of IHD in both genders.

Long-acting nanoparticulate DNase-1 for effective suppression of SARS-CoV-2-mediated neutrophil activities and cytokine storm

Yun Young Lee et al.

Summary: SARS-CoV-2 infection can lead to cytokine storm and multi-organ failure, but exogenously administered long-acting DNase-1 can effectively reduce cfDNA levels and neutrophil activities, potentially serving as a therapeutic intervention for life-threatening SARS-CoV-2-mediated illnesses.

Is Toll-like receptor 4 involved in the severity of COVID-19 pathology in patients with cardiometabolic comorbidities?

Simone Cristina Soares Brandao et al.

Summary: The severe form of COVID-19 is characterized by an abnormal and exacerbated immune response, especially in patients with obesity, diabetes, hypertension, and atherosclerosis. The virus spike protein interacts strongly with TLR4, leading to a cytokine storm and multiple organ failure in some cases.

Effect of Tocilizumab vs Usual Care in Adults Hospitalized With COVID-19 and Moderate or Severe Pneumonia A Randomized Clinical Trial

Olivier Hermine et al.

Summary: In this clinical trial of COVID-19 patients with pneumonia requiring oxygen support but not in the ICU, tocilizumab did not reduce WHO-CPS scores below 5 by day 4 but may have reduced the risk of needing noninvasive ventilation, mechanical ventilation, or death by day 14. There was no difference in mortality by day 28. Further studies are needed to confirm these preliminary results.

Sex differences in COVID-19: candidate pathways, genetics of ACE2, and sex hormones

Anissa Viveiros et al.

Summary: Biological sex is a critical determinant in health and disease, especially in the context of the COVID-19 pandemic where males tend to exhibit more severe symptoms and higher mortality rates. The similarities in sex differences between cardiovascular disease and COVID-19 are largely due to the involvement of the innate immune system and the RAS. Additionally, the role of ACE2 in both diseases and the activation of ADAM17 exacerbating ACE2 loss are key factors influencing disease outcomes.

Impact of cardiovascular disease and risk factors on fatal outcomes in patients with COVID-19 according to age: a systematic review and meta-analysis

SungA Bae et al.

Summary: This study found that cardiovascular disease (CVD) and its risk factors (hypertension and diabetes) were closely related to fatal outcomes in COVID-19 patients of all ages. While younger patients had lower prevalence rates of cardiovascular comorbidities than older patients, the relative risk of fatal outcomes was higher in younger patients with hypertension, diabetes, and CVD compared to older patients.

Genetic mechanisms of critical illness in COVID-19

Erola Pairo-Castineira et al.

Summary: In critically ill patients with COVID-19, host genetic variants associated with antiviral defense mechanisms and inflammatory organ damage were identified through a genome-wide association study. These genetic signals provide potential targets for therapeutic development and repurposing of existing drugs. Large-scale randomized clinical trials will be essential to confirm the effectiveness of targeted treatment strategies.

Repurposed Antiviral Drugs for Covid-19-Interim WHO Solidarity Trial Results

Summary: The authors of the study reported the interim results of using four repurposed antiviral drugs (remdesivir, hydroxychloroquine, lopinavir, and interferon beta-1a) in hospitalized COVID-19 patients. The study showed that these drug regimens had little or no effect on overall mortality, initiation of ventilation, and duration of hospital stay.

Assessment of Neutrophil Extracellular Traps in Coronary Thrombus of a Case Series of Patients With COVID-19 and Myocardial Infarction

Ana Blasco et al.

Summary: Severe COVID-19 can lead to intense formation of neutrophil extracellular traps (NETs) which occlude microvessels, potentially causing ST-elevated myocardial infarction. In patients with COVID-19, a higher density of NETs was found in coronary thrombi compared to historical patients without COVID-19, suggesting a role of NETs in coronary thrombosis pathogenesis. Targeting intravascular NETs could be a relevant treatment goal for preventing coronary thrombosis in severe COVID-19 cases.
