Diagnostic Performance of High-Sensitivity Cardiac Troponin T Strategies and Clinical Variables in a Multisite US Cohort

Summary: In a prospective multisite US cohort, an initial hs-cTnT below the LOQ combined with a low-risk HEART score has a 99% NPV for 30-day MACEs. The 0/1-h hs-cTnT algorithm did not achieve an NPV >99% for 30-day MACEs when used alone or with a HEART score.

Acute Phase Response and Non-Reproducible Elevated Concentrations with a High-Sensitivity Cardiac Troponin I Assay

Summary: High-sensitivity cardiac troponin testing allows for earlier diagnostic and prognostic decisions in the hospital setting. However, caution is needed when interpreting results, as different assays may have varying accuracy, especially in patients with acute phase response.

Late Outcomes of the RAPID-TnT Randomized Controlled Trial 0/1-Hour High-Sensitivity Troponin T Protocol in Suspected ACS

Kristina Lambrakis et al.

Summary: The study compared outcomes of a 0/1-hour high-sensitivity cardiac troponin T (hs-cTnT) protocol with a 0/3-hour masked hs-cTnT protocol in patients with suspected acute coronary syndromes. It found that the use of unmasked hs-cTnT reporting did not reduce ischemic events over 12 months, and may even increase death or myocardial infarction among those with lower initial troponin concentrations.

High-Sensitivity Cardiac Troponin on Presentation to Rule Out Myocardial Infarction

Atul Anand et al.

Summary: The study investigated the safety and efficacy of an early rule-out pathway for patients with suspected acute coronary syndrome. Implementation of this pathway reduced length of stay and increased discharge rate, but did not demonstrate noninferiority for the 30-day safety outcome. However, there was no increase in cardiac events at 1 year.

Disagreement between Cardiac Troponin Tests Yielding a Higher Incidence of Myocardial Injury in the Emergency Setting

Peter A. Kavsak et al.

Summary: Differences in myocardial injury classification between high-sensitivity cardiac troponin assays are largely due to assay design and sensitivity. The OCD hs-cTnI assay shows higher rates of biochemical injury with poorer reproducibility, leading to inconsistent results in different emergency department populations.
