Endoscopic management of pancreatic diseases in patients with surgically altered anatomy: clinical outcomes of combination of double-balloon endoscopy- and endoscopic ultrasound-guided interventions

Summary: This study evaluated the feasibility, effectiveness, and safety of a combination of double-balloon endoscope-assisted ERP and endoscopic ultrasonography-guided pancreatic duct drainage in patients with surgically altered anatomy. Summary: Double-balloon enteroscopy (DBE) is safe and effective for ERCP in patients with surgically altered gastrointestinal anatomy, with success rates of 90%-94% and a 4% adverse event rate. Subgroup analysis did not demonstrate significant differences in outcomes between short-scope and long-scope DBE-ERCP. Further research is needed to clarify the role of DBE compared to other modalities in surgically altered anatomy.

Summary: The study demonstrates the potential of underwater cap-assisted ERCP (u-ERCP) technique in the treatment of biliary disease in patients with surgically altered anatomy (SAA). The case report discussed a patient undergoing EDGI for biliary stone management, which led to dehiscence of the gastro-jejunal anastomosis during the procedure. It highlights the importance of careful evaluation and tailored approach when performing EDGI.

Summary: Treating choledocholithiasis in Roux-en-Y patients is challenging due to altered anatomy, but laparoscopic assisted ERCP is a safe and feasible intervention. Prophylactic cholecystectomy is not recommended in bariatric surgery, and other treatment options exist beyond the traditional methods. The increasing number of patients undergoing bariatric surgery is leading to more common procedures in smaller, non-specialized centers.
