Glial fibrillary acidic protein (GFAP)-antibody in children with focal seizures of undetermined cause

Summary: In a study of 38 children with focal seizures of undetermined cause, two boys were found to have antibodies to glial fibrillary acidic protein (GFAP) even without abnormal brain MRI findings or medical history related to seizures. One boy had chronic treatment-resistant frontal lobe epilepsy, while the other had a single episode of focal seizures followed by cognitive and language impairment without subsequent seizures during a 4-year follow-up period. These results suggest that autoimmune astrocytopathy may play a role in some epilepsy patients and warrant further investigation into its exact mechanism in epileptogenesis.

Summary: In autoimmune GFAP astrocytopathy (GFAP-A), there is a predominance of CD8(+) T cells with marked lymphocytic infiltration, especially adjacent to dystrophic neurons and astrocytes. Additionally, there is diffuse infiltration by CD68(+) and CD163(+) macrophages. CD8(+) astrocytes were identified in some samples, but no CD4(+) astrocytes were observed.
