Effect of Respiratory Physiological Changes on Optic Nerve Sheath Diameter and Cerebral Oxygen Saturation in Patients With Acute Traumatic Brain Injury

Summary: This study evaluated the effects of positive end-expiratory pressure (PEEP) and end-tidal carbon dioxide (ETCO2) on optic nerve sheath diameter (ONSD) and regional cerebral oxygen saturation (rScO2) in patients with acute traumatic brain injury (TBI). The results showed that increasing PEEP led to significant increases in ONSD and rScO2, while reducing ETCO2 significantly decreased ONSD and rScO2.

The main factors associated with increased ICP after PEEP augmentation included reduced Crs, lower MAP, lung recruitment, and increased PaCO2. However, none of these factors was able to predict, at baseline, the ICP response to PEEP.
