Diagnostic accuracy of computed tomography angiography (CTA) for diagnosing blunt cerebrovascular injury in trauma patients: a systematic review and meta-analysis

Charlotte C. Kik et al.

Summary: From 9 studies meeting the inclusion criteria, CTA has reasonable specificity but low sensitivity in diagnosing any BCVI compared to DSA. Increasing the channel to 64 slices did not improve sensitivity. Using only DSA to confirm CTA-positive cases, especially in patients with low-grade injuries, poses a risk of underdiagnosis of BCVI.

Summary: The study analyzed the effectiveness of administering antithrombotic agents to patients with BCVI. It found that antithrombotics reduced the risk of death, while nontreatment increased the risk of stroke and/or death.

Summary: The study compared the risk-benefit profile of antiplatelet (AP) versus anticoagulant (AC) therapy in blunt cerebrovascular injuries (BCVIs) patients. Results showed that the hemorrhage rate was lower in the AP treatment group compared to the AC treatment group, but there was no significant difference in the rate of ischemic stroke between the two treatment groups.

Treatment of asymptomatic blunt cerebrovascular injury (BCVI): a systematic review

Patrick B. Murphy et al.

Summary: This systematic review on the management of asymptomatic BCVI found that any medical management was better than no treatment, but no specific differences were found. Vessel healing was rare and mostly occurred following low-grade injuries.
