Delayed Onset Muscle Soreness (DOMS): The Repeated Bout Effect and Chemotherapy-Induced Axonopathy May Help Explain the Dying-Back Mechanism in Amyotrophic Lateral Sclerosis and Other Neurodegenerative Diseases

Summary: Delayed onset muscle soreness (DOMS) and amyotrophic lateral sclerosis (ALS) are proposed to be caused by acute compression axonopathy of sensory afferents in the muscle spindle, possibly due to genetic/environmental factors, damaging contractions, and neuroinflammatory processes. The dysfunctional microcircuits in these diseases can affect memory and learning, ultimately interfering with life-sustaining central pattern generators in ALS.

Mechanisms of Peripheral and Central Pain Sensitization: Focus on Ocular Pain

Summary: Persistent ocular pain caused by corneal inflammation and nerve injury leads to significant alterations along the pain axis, with changes in both primary sensory neurons in the trigeminal nerves and secondary neurons in the spinal trigeminal nucleus contributing to peripheral and central pain sensitization. Studies using animal models have provided insights into the mechanisms involved in these maladaptive changes. New techniques such as optogenetics and genetic neuronal labelling allow for investigation of identified circuits involved in nociception at the spinal and trigeminal levels. Recent advances in the discovery of molecular and cellular mechanisms contributing to pain sensitization in the trigeminal pathways are also presented.

Summary: The muscle spindle afferents are slowly adapting low threshold mechanoreceptors responsible for reporting muscle length and movement information important for motor control and proprioception. The rapidly adapting cation channel PIEZO2 is essential for muscle spindle afferent stretch sensitivity, but additional molecular elements are required for mediating the complex slowly adapting response of these afferents. Glutamate released from synaptic-like vesicles plays a crucial role in maintaining muscle spindle afferent excitability during stretch, suggesting a potential therapeutic target for normalizing their function.

They highlight the importance of primary damage during strenuous exercises and secondary damage during deceleration. Elevated prostaglandin E2, nitric oxide, and glutamate may play critical roles in the neuro-modulation, leading to mechano-energetic failure and inflammatory events.

Summary: Post orgasmic illness syndrome is a rare condition with unknown pathomechanism and uncertain treatment, characterized by symptoms lasting 2-7 days post ejaculation. Proposed primary injury includes acute compression proprioceptive axonopathy in the muscle spindle and mitochondrial impairment. Similarities to delayed onset muscle soreness exist, but differences arise from spermidine depletion-induced physiological responses. Longitudinally, cognitive and memory consequences are noted due to learning and memory-related primary injury.
