Summary: In pediatric patients with functional constipation, lubiprostone did not show statistically significant effectiveness over placebo, but exhibited a safety profile similar to that in adults. Both doses of lubiprostone were well tolerated in children and adolescents.

Dietary interventions for functional abdominal pain disorders in children: a systematic review and meta-analysis

Clara M. A. de Bruijn et al.

Summary: This systematic review assessed the efficacy and safety of dietary interventions for pediatric functional abdominal pain disorders (FAPDs). The results suggest that the use of fibers may lead to higher treatment success, but the quality of evidence is very low. Further high-quality intervention trials are needed to investigate the effectiveness of other dietary interventions.

Summary: This article systematically reviews the outcomes of cognitive behavioral therapy (CBT), educational support, yoga, hypnotherapy, gut-directed hypnotherapy, guided imagery, and relaxation in the management of functional abdominal pain disorders (FAPDs) in children. The results suggest that CBT and hypnotherapy may be considered as effective treatments for FAPDs in childhood.

Low FODMAP Diet Is Not Effective in Children with Functional Abdominal Pain: A Randomized Controlled Trial

Summary: The pilot study suggests that the low FODMAP diet is not effective in reducing symptoms in children with FAP. The NICE diet group experienced significant improvement in abdominal pain and stool consistency. No statistical significance was found in the improvement of abdominal symptoms in the low FODMAP group.

ACG Clinical Guideline: Management of Irritable Bowel Syndrome

Brian E. Lacy et al.

Summary: Irritable bowel syndrome (IBS) is a chronic disorder that significantly impacts patients' quality of life. The American College of Gastroenterology has developed a clinical guideline using GRADE methodology for the management of IBS, recommending a positive diagnostic approach, specific tests, treatments, and gut-directed psychotherapy to improve symptoms for patients with IBS.

Markers of Bile Acid Metabolism in Pediatric Diarrhea Predominant Irritable Bowel Syndrome and Healthy Controls

Beate C. Beinvogl et al.

Summary: This study aimed to investigate the association of bile acid levels with bile acid diarrhea in pediatric IBS-D patients, with 20% of pediatric IBS-D patients showing abnormal fasting serum C4 levels and 28% having low FGF-19 values. Fecal primary bile acids were significantly correlated with stool frequency, suggesting that these biomarkers may help diagnose bile acid diarrhea in pediatric IBS-D.

Internet-delivered cognitive behavioral therapy for youth with functional abdominal pain: a randomized clinical trial testing differential efficacy by patient subgroup

Lynn S. Walker et al.

Summary: The results of this study suggest that subgrouping pediatric FAP patients based on pain-related psychological characteristics can significantly impact the effectiveness of cognitive behavior therapy. Specifically, adolescents classified as high pain dysfunctional (HPD) showed greater reduction in GI symptoms and abdominal pain with CBT compared to those classified as high pain adaptive (HPA) and low pain adaptive (LPA). This research highlights the importance of personalized treatment approaches based on patient subgroups in optimizing treatment outcomes for pediatric FAP.

Electrocardiogram Before Tricyclic Antidepressant Use: Minimal Impact in Pediatric Functional Gastrointestinal Disorders

Lauren J. Klein et al.

Summary: This study aimed to determine the effect of electrocardiogram findings on the initiation of tricyclic antidepressants for functional gastrointestinal disorders and evaluate cardiac outcomes related to low dose TCA use. It found that screening ECGs infrequently influence TCA initiation and may lead to increased resource utilization, while serious adverse cardiac events are unlikely with low dose TCA administration.

Pharmacologic Treatment in Functional Abdominal Pain Disorders in Children: A Systematic Review

Robyn Rexwinkel et al.

Summary: Based on the current evidence, it is not possible to recommend any specific pharmacologic agent for the treatment of pediatric FAPDs. However, agents such as antispasmodics or antidepressants can be discussed in daily practice because of their favorable treatment outcomes and the lack of important side effects. High-quality RCTs are necessary to provide adequate pharmacologic treatment.

Summary: The efficacy and safety of antidepressants for Functional Abdominal Pain Disorders (FAPDs) in children and adolescents may not differ significantly from placebo. However, the current evidence is insufficient to support clinical decision-making regarding the use of these medications. Further studies with larger sample sizes and longer follow-up are needed to provide more conclusive results.
