A 33-year follow-up after valvular surgery for carcinoid heart disease

Johannes Bergsten et al.

Summary: This study reviewed the survival and echocardiographic findings after valve surgery for severe CaHD. Survival was significantly higher in patients who underwent combined tricuspid valve replacement (TVR) and pulmonary valve replacement (PVR) compared with those who only had TVR or TVR with pulmonary valvotomy (no PVR). Bioprosthetic valves showed good durability. Preoperative echocardiography may underestimate pulmonary pathology. Combined TVR and PVR should be considered for most patients.

Biomarkers in Small Intestine NETs and Carcinoid Heart Disease: A Comprehensive Review

Markos Kalligeros et al.

Summary: Neuroendocrine tumors (NETs) are a heterogeneous group of tumors that present diagnostic and therapeutic challenges. Biomarkers have been traditionally used for diagnosis and management, with traditional monoanalyte biomarkers having limitations due to tumor heterogeneity and non-secretory nature. The development of new multianalyte biomarkers, such as NETest, has revolutionized the field by allowing for real-time disease status assessment through liquid biopsy.

Update in carcinoid heart disease-the heart of the matter

Kira Oleinikov et al.

Summary: Carcinoid heart disease is a paraneoplastic cardiac manifestation that occurs in patients with carcinoid syndrome and advanced neuroendocrine malignancy, leading to right-sided valvular fibrosis and heart failure. Early diagnosis and timely surgical intervention, especially valve replacement surgery, are important for improving survival outcomes in patients with advanced disease. Collaboration of a multidisciplinary team with experience is essential for optimal management of patients with CHD.

Progression of Carcinoid Heart Disease in the Modern Management Era

Emilie Baron et al.

Summary: The prevalence and natural history of carcinoid heart disease (CaHD) may vary over time, with an increase in urinary 5-hydroxyindoleacetic acid identified as an independent predictor of CaHD occurrence. Regular cardiac follow-up is emphasized for patients with recurrence of the tumor process.

Comparison of Serum and Urinary 5-Hydroxyindoleacetic Acid as Biomarker for Neuroendocrine Neoplasms

Anna Becker et al.

Summary: The study evaluated an improved tandem mass spectrometric serum 5HIAA assay for the diagnosis and follow-up of NEN, showing that the serum assay performed equally well as the urinary assay. The outcome and sensitivity for serum and 24-h urine assays were convergent, with serum instead of 24-h urine 5HIAA recommended for NEN patients due to its reliability and convenience.

Serotonin: a platelet hormone modulating cardiovascular disease

Marina Rieder et al.

Summary: Cardiovascular diseases and depression are significant health burdens, with increasing evidence showing a causal relationship between them. Serotonin 5-hydroxytryptamin (5-HT) plays a role in the pathogenesis of both conditions as a neurotransmitter and peripheral hormone. Further research is needed to understand the clinical effects of 5-HT in specific tissues for targeted pharmacological therapies.

Summary: PRRT with Lu-177-DOTATATE effectively reduced diarrhea and flushing in patients with carcinoid syndrome, showing potential as a symptomatic treatment option for patients whose symptoms are not well controlled with somatostatin analogs.
