Neonatal platelet physiology and implications for transfusion

Francisca Ferrer-Marin et al.

Summary: The neonatal hemostatic system differs from that of adults, with developmental differences in platelet function and primary hemostasis. Factors in neonatal blood such as high von Willebrand factor levels increase platelet-vessel wall interaction, leading to shorter bleeding times in healthy full-term infants. However, prophylactic platelet transfusions do not decrease bleeding in preterm neonates and may increase neonatal morbidity and mortality.

Heightened activation of embryonic megakaryocytes causes aneurysms in the developing brain of mice lacking podoplanin

Christopher Hoover et al.

Summary: In mice, the ligand PDPN for CLEC-2 is temporarily expressed in neural tissue during midgestation and its loss leads to aneurysms and spontaneous hemorrhage, with enhanced granule release and localization of eMks/platelets in the lower diencephalon before hemorrhage. PDPN counteracted collagen-1-induced secretion of angiopoietin-1, leading to enhanced TIE-2 activation in aneurysm-like sprouts of PDPN-deficient embryos, and blocking platelet activation prevented vascular defects in the PDPN-deficient embryos.

Age-Dependent Control of Collagen-Dependent Platelet Responses by Thrombospondin-1-Comparative Analysis of Platelets from Neonates, Children, Adolescents, and Adults

Katrin Herken et al.

Summary: The study found that neonatal platelets lack responsiveness to thrombospondin-1, resulting in a relatively impaired response to collagen. Platelet responsiveness varied with age, agonist, and activation marker among different pediatric age groups.

Variation in Neonatal Transfusion Practice

Ravi M. Patel et al.

Summary: This study estimated the incidence of blood product transfusion and characterized pretransfusion hematologic values for infants during their initial hospitalization after birth. Results showed wide variability in pretransfusion hemoglobin, platelet count, and international normalized ratio values for neonatal transfusions, suggesting room for improved patient blood management in the US.

Platelet Transfusion and Outcomes of Preterm Infants: A Cross-Sectional Study

Marwa M. Elgendy et al.

Summary: The study found that prophylactic platelet transfusion in preterm infants is associated with increased mortality and morbidity, especially in infants with lower birth weight. There is a trend of increased utilization of platelet transfusions over the study period.

Platelet Innate Immune Receptors and TLRs: A Double-Edged Sword

Theo Ebermeyer et al.

Summary: Platelets play a crucial role in maintaining vascular integrity and hemostasis. Additionally, they interact with immune cells to modulate immune responses. Through various receptors, platelets are involved in physiological processes, including those associated with pathogens and diseases.
