Isolated Diastolic Hypertension and Risk of Cardiovascular Events: A Systematic Review and Meta-Analysis of Cohort Studies With 489,814 Participants

Summary: This meta-analysis demonstrates that isolated diastolic hypertension (IDH), as defined by the 2018 ESC criterion, is significantly associated with an increased risk of composite cardiovascular events, cardiovascular mortality, all strokes (including ischemic and hemorrhagic stroke), and hemorrhagic stroke specifically. However, IDH is not significantly associated with all-cause mortality and ischemic stroke. Subgroup analysis further reveals that the association between IDH and composite cardiovascular events is significant in younger adults and Asians, but not in elderly patients, Americans, and Europeans.

Summary: This study showed that combined treatment with a polypill plus aspirin reduced cardiovascular events among participants without cardiovascular disease but at intermediate cardiovascular risk. However, the polypill alone or aspirin alone did not significantly reduce cardiovascular events. Participants in the polypill group had a higher incidence of hypotension or dizziness compared to the placebo group.

Summary: By pooling raw data from four large observational studies, it was found that switching to a Per/Ind single-pill combination led to rapid and effective decreases in blood pressure in patients with T2DM, obesity, or MetS, achieving blood pressure control in previously uncontrolled patients. The treatment was well tolerated, without specific safety concerns raised.

Long-Term Impact of Different Triple Combination Antihypertensive Medications on Blood Pressure Control, Metabolic Pattern and Incident Events: Data from the Brisighella Heart Study

Summary: This study aimed to evaluate the long-term effects of different triple combination antihypertensive medications in an Italian cohort. Patients treated with Perindopril/Amlodipine/Indapamide showed better blood pressure control, stable lipid profile, and no age-related increase in cholesterol levels. Combination treatment with RAS modulators, amlodipine, and thiazides was more effective in lowering diastolic BP and had a positive impact on serum lipids.

Summary: The study evaluated the prevalence and predictors of ambulatory blood pressure control in patients taking a triple antihypertensive therapy containing an ACEi or an ARB. Patients on triple therapy with ACEi had a higher prevalence of ambulatory blood pressure control, which was affected by the lower number of antihypertensive pills taken. The importance of fixed-dose combinations in the management of essential hypertension was confirmed.

Adherence to Lipid-Lowering Treatment by Single-Pill Combination of Statin and Ezetimibe

Trial of Intensive Blood-Pressure Control in Older Patients with Hypertension

Summary: In 8511 Chinese patients aged 60 to 80 with hypertension, intensive treatment with a systolic blood-pressure target of 110 to <130 mm Hg resulted in a lower incidence of cardiovascular events compared to standard treatment with a target of 130 to <150 mm Hg. Except for a higher incidence of hypotension, the two groups had similar rates of adverse events.
