The fecal mycobiome in non-alcoholic fatty liver disease

Summary: The study found that the fecal fungal composition of patients with non-alcoholic fatty liver disease (NAFLD) is associated with disease severity. Non-obese NAFLD patients have a distinct fungal ratio in their feces compared to other types of patients. Antifungal treatment in a mouse model can reduce liver damage from steatohepatitis.

Summary: CAID is a spectrum of immune alterations associated with end-stage liver disease, including systemic inflammation and immune deficiency. The severity of CAID progresses parallel with cirrhosis stage, involving low-grade and high-grade systemic inflammatory phenotypes. Therapies targeting modulation of this dysfunctional immune response are currently under evaluation in preclinical and clinical studies.

Summary: This study found that healthy donor fecal microbiota transplantation (FMT) is associated with significantly lower incidence of ascites, infections, encephalopathy, and alcohol relapse, as well as higher survival rates, in patients with severe alcohol-associated hepatitis (SAH) compared to standard of care (SoC). Furthermore, FMT was shown to have beneficial modulation of gut microbiota (GM).

Summary: CRIg expressed on liver macrophages mediates phagocytosis of Gram-positive bacteria, but ethanol impairs its function, leading to progression of alcoholic liver disease. Deficiency in CRIg results in reduced clearance of Gram-positive bacteria in the liver, while administration of CRIg-Ig protein can protect mice from ethanol-induced steatohepatitis.

Further research is needed to explore the structure, composition, and potential regulatory functions of human GALT in intestinal immune responses.

Summary: In this multicenter controlled trial, G-CSF did not show significant beneficial effects on ACLF patients compared to standard medical therapy alone. The study was terminated prematurely due to futility after analysis. Therefore, G-CSF should not be recommended as a standard treatment for ACLF.

Dynamic Changes of the Fungal Microbiome in Alcohol Use Disorder

Summary: The study revealed dynamic changes in the intestinal fungal microbiome in patients with alcohol use disorder (AUD), and demonstrated that improved liver health after alcohol abstinence in AUD patients was associated with lower abundances of specific fungal genera and species, as well as changes in serum anti-fungal IgG levels. These findings suggest that intestinal fungi could be a potential therapeutic target to improve the outcome of patients with ALD.
