Preoperative chemotherapy in clinically node positive muscle invasive bladder cancer: Radiologic variables can predict response

Summary: A study was conducted to evaluate the impact of pathologic downstaging after radical cystectomy and pelvic lymph node dissection on clinically lymph node positive urothelial bladder cancer, as well as to identify optimal preoperative imaging variables for predicting pathologic nodal status. Results showed that post-chemotherapy CT scans with an 8 mm nodal size cut-off could be a better predictor of pathologic nodal status than traditional measures. Patients with clinically node positive bladder cancer may experience significant pN0 after induction chemotherapy, with implications for survival outcomes.
