Unique Cholangiocyte-Targeted IgM Autoantibodies Correlate With Poor Outcome in Biliary Atresia

Summary: The study found that biliary atresia is associated with cholangiocyte-predominant IgM autoantibodies in the first year after the Kasai portoenterostomy. Anti-CHI3L1, anti-DLL-4, and anti-SFTPD IgM autoantibodies correlated with worse outcomes, and detection of C3d on cholangioctyes and antigen-specific autoreactive T cells suggests that autoimmunity plays a role in ongoing bile duct injury and disease progression.

Summary: A systematic review of studies evaluating biomarkers for BA diagnosis and post-KPE prognosis showed that MMP-7, IL-33, and GGT are useful for diagnosing BA, while APRi can predict liver fibrosis and cirrhosis after KPE. These noninvasive biomarkers can be integrated into BA management protocols.
