Global burden of bacterial antimicrobial resistance in 2019: a systematic analysis

Christopher J. L. Murray et al.

Summary: Antimicrobial resistance (AMR) poses a significant threat to global human health, and this study provides the most comprehensive estimates of AMR burden to date. By estimating deaths and disability-adjusted life-years (DALYs) associated with bacterial AMR in 204 countries and territories in 2019, the study highlights the impact of resistance and the leading pathogen-drug combinations contributing to it.

Antimicrobial stewardship for surgical antibiotic prophylaxis and surgical site infections: a systematic review

Joselin Valeska Martinez-Sobalvarro et al.

Summary: AMS, including audit, feedback, education, implementation of a protocol, and computer-assisted decision support, have shown effectiveness in promoting adherence to surgical antibiotic prophylaxis protocols, reducing surgical site infection rate, and providing positive economic impacts. However, more randomized clinical trials are needed to improve the evidence level for decision-making.

Use of Feedback Data to Reduce Surgical Site Infections and Optimize Antibiotic Use in Surgery A Systematic Scoping Review

Shalini Ahuja et al.

Summary: This study conducted a systematic scoping review of 21 studies on surgical site infection and antibiotic usage, finding that feedback plays a key role in reducing SSI rates and optimizing antibiotic usage. However, the optimal method, format, and frequency of feedback remain unclear and require further research and exploration.

A national, multicentre, web-based point prevalence survey of antimicrobial use and quality indices among hospitalised paediatric patients across South Africa

P. P. Skosana et al.

Summary: Data on antimicrobial consumption among paediatric population in public hospitals in South Africa is limited. This study aimed to quantify antimicrobial usage, identify and classify which antimicrobials are used based on the WHO AWaRe list. The study found that antimicrobial prescribing is common among paediatric patients in South Africa and interventions should be targeted to improve prescribing practices and encourage greater use of oral antibiotics.

Point Prevalence Survey of Antibiotic Use across 13 Hospitals in Uganda

Reuben Kiggundu et al.

Summary: This study assessed antibiotic use in hospitals in Uganda using the standardized point prevalence survey methodology recommended by the World Health Organization. The findings revealed gaps in antibiotic usage, such as low compliance with clinical guidelines and excessive use of watch-classified antibiotics. The results of this study are important benchmarks for improving antimicrobial stewardship in Uganda and guiding future evaluations.

Tackling antimicrobial resistance across sub-Saharan Africa: current challenges and implications for the future

Brian Godman et al.

Summary: All sub-Saharan African countries have developed their National Action Plans (NAPs), but implementation varies. Key challenges include personnel, expertise, capacity, and resources, as well as the lack of focal points and competing demands. Coordinated efforts are needed to address these challenges and reduce AMR.

Strategies to Improve Antimicrobial Utilization with a Special Focus on Developing Countries

Brian Godman et al.

Summary: Antimicrobial resistance is a global concern with initiatives focusing on improving antibiotic use, strengthening antimicrobial stewardship programs, enhancing healthcare facility surveys, and intensifying public health education efforts. Furthermore, new quality indicators need to be developed for monitoring medication use in ambulatory care settings.

Key Issues Surrounding Appropriate Antibiotic Use for Prevention of Surgical Site Infections in Low- and Middle-Income Countries: A Narrative Review and the Implications

Julius C. Mwita et al.

Summary: There are significant concerns with the timing and duration of surgical antibiotic prophylaxis in LMICs. Successful interventions involve multiple approaches such as education, monitoring, and setting quality targets, which can improve appropriateness, reduce prolonged duration, and decrease costs. Multiple interventions among LMICs can address these concerns and provide future directions.

A point prevalence survey of antimicrobial utilisation patterns and quality indices amongst hospitals in South Africa; findings and implications

P. P. Skosana et al.

Summary: The study found a high level of antimicrobial use in public sector hospitals in South Africa, with penicillins being the most commonly prescribed group. 33.6% of patients received antimicrobial treatment, with Access antimicrobials being mostly used in medical and surgical wards, and Watch antimicrobials in the ICU. Compliance with treatment guidelines was good, but there were concerns about extended use of antimicrobials for surgical prophylaxis.

Point Prevalence Survey of Antimicrobial Utilization in Ghana's Premier Hospital: Implications for Antimicrobial Stewardship

Daniel Ankrah et al.

Summary: The first comprehensive point prevalence survey conducted at the Korle Bu Teaching Hospital found that community-acquired infections were more commonly treated empirically, with antibiotics mainly prescribed for pneumonia, skin and soft tissue infections, and sepsis. While guideline compliance was high, improvement is needed in completing stop/review dates for prescriptions.

Antibiotic Prescribing Patterns in Ghana, Uganda, Zambia and Tanzania Hospitals: Results from the Global Point Prevalence Survey (G-PPS) on Antimicrobial Use and Stewardship Interventions Implemented

Nikki D'Arcy et al.

Summary: Antimicrobial resistance is a significant global public health issue, and the Global Point Prevalence Survey conducted in 17 hospitals across four African countries revealed that most antimicrobial prescriptions belong to the WHO 'Access' and 'Watch' categories. The study also found low prescription rates for 'Reserve' category antibiotics and emphasized the importance of ongoing monitoring and broader participation in improving antimicrobial stewardship programs.

Assessment of adherence to pneumonia guidelines and its determinants in an ambulatory care clinic in Ghana: findings and implications for the future

Israel Abebrese Sefah et al.

Summary: This study evaluated adherence to the Ghana Standard Treatment Guidelines for the empirical antibiotic treatment of ambulatory care patients with community-acquired pneumonia in a region in Ghana, and found a low adherence rate. More female and pediatric patients were observed, with a high percentage having valid national health insurance. Adherence was associated with antibiotic prescribing duration, the number of additional antibiotics prescribed, and some patients' clinical characteristics.

Summary: An educational intervention combined with daily audit and feedback to clinicians at Mbarara Regional Referral Hospital maternity ward in Uganda increased the utilization of the WHO surgical safety checklist and pre-operative antibiotic administration for cesarean delivery. However, the rates of adherence decreased over time, highlighting the need for further research and implementation interventions to sustain usage and impact on clinical outcomes.
