Clinico-radiological characteristics of anti-myelin oligodendrocyte glycoprotein antibody-associated autoimmune encephalitis in children

Ji Yeon Han et al.

Summary: This study investigated the clinical characteristics and prevalence of paediatric anti-MOG antibody-associated autoimmune encephalitis. The results showed that anti-MOG antibody-associated encephalitis accounts for a significant proportion of paediatric patients with autoimmune encephalitis. Patients with this disease had a favorable response to immunotherapy.

Clinical analysis of myelin oligodendrocyte glycoprotein antibody-associated demyelination in children: A single-center cohort study in China

Xin Wang et al.

Summary: The clinical phenotypes of MOGAD are age-dependent, with younger onset ages for ADEM compared to ON children. The cerebral lesions are diverse and can manifest as ADEM-like, leukodystrophy-like, ON, and other patterns. The titers of MOG-Ab should not be the sole basis for recurrence and long-term treatment. Most children have a good prognosis, but ADEM-ON phenotype tends to relapse, sometimes leading to cognitive impairment, epilepsy, and other sequelae. Repeated ON can cause visual impairment.

Summary: The study evaluated the application of antibody prevalence and response to immunotherapy in Chinese patients with epilepsy and encephalopathy, finding that APE2-CHN had the highest value for predicting the positivity of NMDAR-Ab. Additionally, RITE2-CHN appropriately evaluated the response of immunotherapy for anti-LGI1 encephalitis. However, RITE2 and RITE2-CHN did not appear to be good predictors of immunotherapy outcomes for patients with specific neuronal-surface antibodies, and high APE2-CHN scores often indicated a poor response to immunotherapy.

MRI abnormalities were observed in all patients, with the most common lesion locations being cortical/subcortical, deep/periventricular white matter, and corpus callosum. Summary: Anti-NMDA receptor encephalitis (NMDAR-E) is an autoimmune encephalitis mainly affecting young females, often presenting with psychiatric symptoms that can overlap with preexisting psychiatric disorders. This case study highlights the importance of being vigilant for neuropsychiatric warning signs in patients with preexisting psychiatric illness in order to make a timely diagnosis and initiate treatment. Additionally, it emphasizes the recognition of overlap syndromes, such as the coexistence of NMDAR-E and demyelinating disorders, and the complicated diagnostic and therapeutic implications associated with these conditions.
