Guidelines for Genetic Testing and Management of Alport Syndrome

Judy Savige et al.

Summary: Genetic testing for pathogenic COL4A3?5 variants is crucial for investigating persistent hematuria and suspected inherited kidney diseases. Even in cases where a heterozygous pathogenic COL4A3 or COL4A4 is suspected, experts recommend genetic testing and cascade testing of family members to reduce the risk of kidney dysfunction. Testing for variants in the COL4A3?5 genes is also recommended for other kidney conditions such as proteinuria, steroid-resistant nephrotic syndrome, IgA glomerulonephritis, and kidney failure of unknown cause.

NanoLuc reporters identify COL4A5 nonsense mutations susceptible to drug-induced stop codon readthrough

Summary: Experimental results suggest that PTC readthrough therapy is a feasible approach for a fraction of patients with Alport syndrome, as readthrough drugs can facilitate the functional restoration of coding products from certain nonsense variants.

Genetic Etiologies for Chronic Kidney Disease Revealed through Next-Generation Renal Gene Panel

Anthony J. Bleyer et al.

Summary: The use of broad panel genetic testing by clinical nephrologists had a high success rate, similar to results obtained by academic centers specializing in genetics. Positive genetic findings were identified in 21.1% of cases, with the most common gene variants being PKD1, COL4A5, PKD2, COL4A4, COL4A3, and TTR.

Genetic Basis of Type IV Collagen Disorders of the Kidney

Catherine Quinlan et al.

Summary: The glomerular basement membrane, primarily composed of type IV collagen, plays a vital role in kidney filtration. Specific isoforms of type IV collagen are essential for normal membrane function and mutations can lead to conditions such as Alport syndrome. Patients with mutations affecting type IV collagen exhibit wide variability in phenotype.

Judy Savige et al.

Summary: The meeting expanded the indications for screening for pathogenic variants in the COL4A5, COL4A3 and COL4A4 genes, refined the ACMG criteria for variant assessment, and identified mutational hotspots in the collagen IV alpha 5, alpha 3 and alpha 4 chains.

Prevalence Estimates of Predicted Pathogenic COL4A3-COL4A5 Variants in a Population Sequencing Database and Their Implications for Alport Syndrome

Joel Gibson et al.

Summary: This study estimated the frequencies of predicted pathogenic COL4A3-COL4A5 variants in populations without known kidney disease. The COL4A5 variants were found in at least one in 2320 individuals, and the predicted pathogenic heterozygous COL4A3 and COL4A4 variants affected one in 106 of the population. The results suggest the population frequencies for Alport syndrome need to be adjusted for individual variant penetrance and other factors.

Tyne IV Collagen Variants in CKD: Performance of Computational Predictions for Identifying Pathogenic Variants

Cole Shulman et al.

Summary: The study evaluated the performance of in silico programs in classifying COL4A3/A4/A5 variants, finding that these programs are sensitive but not specific in assigning pathogenicity, leading to misclassification of benign and uncertain significance variants. It is recommended to follow more objective levels of evidence suggested by medical genetics guidelines.
