Comparison of isotonic and hypotonic intravenous fluids in term newborns: is it time to quit hypotonic fluids

Funda Tuzun et al.

Summary: This study compared the use of isotonic and hypotonic fluids for hospitalized term newborns and found that hypotonic fluids may lead to unsafe decreases in plasma sodium levels. However, larger studies are still needed to determine the safety of using hypotonic fluids in term newborns.

Comparison of isotonic versus hypotonic intravenous fluid for maintenance fluid therapy in neonates more than or equal to 34 weeks of gestational age - a randomized clinical trial

Krishna Dathan et al.

Summary: This study compared isotonic and hypotonic intravenous fluid for maintenance therapy in neonates, and found that hypotonic fluid did not show a significant advantage in reducing the incidence of hyponatremia in neonates.

Risk of Overcorrection in Rapid Intermittent Bolus vs Slow Continuous Infusion Therapies of Hypertonic Saline for Patients With Symptomatic Hyponatremia The SALSA Randomized Clinical Trial

Seon Ha Baek et al.

Summary: This study compared the risk of overcorrection in rapid intermittent bolus (RIB) and slow continuous infusion (SCI) therapy with hypertonic saline for symptomatic severe hyponatremia. Results showed no significant difference in overcorrection risk between the two therapies, but RIB had a lower incidence of therapeutic relowering treatment and tended to be more effective in achieving target correction rate within 1 hour than SCI.

Risk of Electrolyte Disorders in Acutely Ill Children Receiving Commercially Available Plasmalike Isotonic Fluids A Randomized Clinical Trial

Saara Lehtiranta et al.

Summary: In a randomized clinical trial conducted at the pediatric emergency department of Oulu University Hospital in Finland, commercially available plasmalike isotonic fluid therapy significantly increased the risk of clinically significant electrolyte disorders, especially hypokalemia, in acutely ill children compared to previously widely used moderately hypotonic fluid therapy containing 20 mmol/L of potassium.

Fluid Balance in the Critically Ill Child Section: How Bad Is Fluid in Neonates?

Austin Rutledge et al.

Summary: Fluid overload (FO) in neonates is a understudied area, but recent research suggests that neonates are susceptible to developing FO, posing challenges to fluid balance. FO not only has deleterious impacts on other organ systems, but may also affect survival.

Summary: The study showed that switching to isotonic intravenous maintenance fluids was associated with a decrease in the prevalence of hyponatraemia. However, hyponatraemia still occurred in over 20% of patients.
