Patients with neutrophilic COPD are susceptible to significant microbiome shifts during exacerbations, especially those in a stable state at baseline. Monitoring the temporal variability of the airway microbiome may provide insight into patient inflammatory status over time.

Summary: The study found that approximately 25% of COPD patients were colonised with Haemophilus influenzae, while colonisation by other bacterial species was less common. Colonisation with Haemophilus influenzae was associated with increased neutrophil counts in sputum, and eosinophilic inflammation rarely coexisted with Haemophilus influenzae colonisation.

Small airway determinants of airflow limitation in chronic obstructive pulmonary disease

Summary: In patients with COPD, thickening of epithelial and subepithelial tissue, mucus plugging, reduced collagen density in the airway wall, and loss of radial alveolar attachments were associated with airflow limitation. The major determinant of airflow limitation related to small airways was found to be the loss of alveolar attachments, which may be mediated by neutrophilic inflammation. Additionally, quantitative analysis of CT scans found an association between the extent of emphysema and loss of alveolar attachments.
