Impact of Diastolic Vessel Restriction on Quality of Life in Symptomatic Myocardial Bridging Patients Treated With Surgical Unroofing: Preoperative Assessments With Intravascular Ultrasound and Coronary Computed Tomography Angiography

Summary: Despite optimal medical therapy, myocardial bridge (MB) can lead to life-limiting symptoms in some patients. In patients with symptomatic MB undergoing surgical unroofing, preoperative IVUS and coronary computed tomography angiography can help identify determinants of preoperative symptoms and improvements in symptoms after surgery. Vessel restriction in diastole, rather than the degree of systolic compression or extent of the MB, appears to be the primary determinant of clinical symptoms and quality of life improvement following surgical unroofing.

While traditionally considered benign, more attention is now being focused on specific subsets of MB associated with ischemic symptoms.
