Machine learning approach in diagnosing Takotsubo cardiomyopathy: The role of the combined evaluation of atrial and ventricular strain, and parametric mapping

Left Atrial Strain for Assessment of Left Ventricular Diastolic Function Focus on Populations With Normal LVEF

Summary: Left atrial strain, especially reservoir strain, has been suggested as a stand-alone measurement for assessing left ventricular diastolic function and estimating filling pressures. However, recent data indicate challenges in using this parameter in patients with normal ejection fraction due to wide normal value range and load dependency of left atrial strain. The accuracy of left atrial reservoir strain for estimating filling pressures is higher in patients with depressed ejection fraction compared to those with normal ejection fraction. Promising data show an association between left atrial reservoir strain and outcome events in patients with normal ejection fraction, but further studies are needed to confirm its incremental value over clinical and other echocardiographic measurements.

Prognostic Significance of Cardiac Magnetic Resonance in Left Atrial and Biventricular Strain Analysis during the Follow-Up of Suspected Myocarditis

Summary: This retrospective study aims to assess the variation in left atrial and biventricular strain and its prognostic value in suspected myocarditis. The results showed that the left atrial total emptying fraction was the only functional index that showed a statistically significant improvement, and the initial left atrial peak's late negative strain rate was the only parameter with significant predictive power of major adverse cardiac events. Therefore, cardiac magnetic resonance strain has great potential for monitoring left atrial and ventricular function in suspected myocarditis.

Pathophysiological Mechanisms of Cardiac Dysfunction in Transgenic Mice with Viral Myocarditis

Summary: Using a transgenic mouse strain (TG) containing a CVB3 Delta VP0 genome, we have revealed virus-mediated cardiac pathophysiological processes in vivo and in vitro. Transgenic mice showed significant alterations in cardiac function, pathologic ECG alterations, calcium homeostasis, intracellular organization, and gene expression. The results also suggest that mitochondrial impairment may contribute to cardiac contractile dysfunction.

Left atrial strain parameters derived by echocardiography are impaired in patients with acute myocarditis and preserved systolic left ventricular function

Summary: This study investigated the diagnostic value of speckle tracking echocardiography in patients with acute myocarditis and normal left ventricular ejection fraction (LVEF). The results showed that patients with acute myocarditis had impaired left ventricular and left atrial function compared to healthy controls, despite preserved LVEF. This suggests that ventricular diastolic dysfunction and elevated LV filling pressures may be present in these patients.

Summary: This study aimed to differentiate between Takotsubo syndrome (TS) and acute myocarditis (AM) using cardiac magnetic resonance imaging coupled with tissue-tracking technique (CMR-TT) and parametric mappings analysis. The results showed that basal radial strain (RS) and apical tissue mapping analysis were the most effective CMR-derived parameters in distinguishing between TS and AM.

The findings suggest that Takotsubo patients may have impaired bi-atrial strain, particularly in the reservoir and conduit functions. These findings highlight the potential role of CMR-FT in characterizing Takotsubo cardiomyopathy.

Summary: This study compared the strain and strain rate parameters of the left and right atrium in patients with Takotsubo and acute myocarditis by using cardiac magnetic resonance. The results showed that patients with Takotsubo had significantly decreased left atrium strain and strain rate parameters compared to patients with acute myocarditis. The assessment of left atrium strain may play a role in distinguishing between Takotsubo and acute myocarditis.

However, there was no significant difference in the change of segmental strain parameters between LGE+ and LGE- segments.
