In Vitro and In Vivo Synergism of Fosfomycin in Combination with Meropenem or Polymyxin B against KPC-2-Producing Klebsiella pneumoniae Clinical Isolates

Summary: In this study, the activity of fosfomycin in combination with meropenem or polymyxin B against KPC-KPN was evaluated. The results showed that the combination of fosfomycin and meropenem restored meropenem activity and exhibited a synergistic effect with fosfomycin and polymyxin B. In the Galleria mellonella model, the combination of fosfomycin and meropenem had a higher survival rate for larvae, while fosfomycin and polymyxin B combination had a more significant biofilm disruption effect.

A novel KPC-113 variant conferring carbapenem and ceftazidime- avibactam resistance in a multidrug-resistant Pseudomonas aeruginosa isolate

Summary: This study characterized a novel KPC-113 variant in a clinical Pseudomonas aeruginosa isolate R20-14. Genomic analysis revealed a plasmid carrying blaKPC-113, which conferred both carbapenem and ceftazidime-avibactam resistance. KPC-113 presented reduced catalytic ability and poor response to avibactam inhibition compared to KPC-2. Efforts should be made to prevent the horizontal spread of blaKPC-113 in clinical settings.

Summary: We studied an outbreak of polymyxin B (PB)- and carbapenem-resistant Klebsiella pneumoniae during the COVID-19 pandemic and found that all isolates showed resistance to multiple drugs, with one particular subtype spreading in the ICU. This outbreak was related to the collapse of the local healthcare system and resulted in high mortality rates.

Klebsiella pneumoniae Carbapenemase Variants Resistant to Ceftazidime-Avibactam: an Evolutionary Overview

Summary: This review describes the mutational landscape of KPC beta-lactamase towards ceftazidime-avibactam resistance, showing that resistance is associated with three hot spots and a high representation of insertions and deletions compared to other beta-lactamases. The extension of resistance to ceftazidime-avibactam is accompanied by a trade-off in resistance to other beta-lactams and a decrease in enzyme stability. The natural stability of KPC could explain its propensity to acquire in vivo mutations conferring resistance to ceftazidime-avibactam.

Risk factors for carbapenem-resistant Enterobacterales infection among hospitalized patients with previous colonization

Summary: This study aimed to identify the risk factors for subsequent carbapenem-resistant Enterobacterales (CRE) infections in patients with initial rectal colonization with CRE. A retrospective case-control study was conducted, and it was found that more than three times hospitalization, being in a coma, and exposure to carbapenems were independent risk factors for CRE clinical infection among CRE rectal carriers.

Summary: The study found that a significant portion of KPC-producing Klebsiella pneumoniae isolates in a Brazilian hospital exhibited resistance to common aminoglycosides like gentamicin and amikacin, with the presence of the rmtB gene in 30% of the strains. The rmtB gene was mainly found in sequence type 258 (ST258) and a novel ST16 clone, carried on large IncC plasmids. Furthermore, most isolates remained susceptible to the veterinary approved drug apramycin. The dissemination of rmtB in an endemic setting for KPC-producing clones is concerning, as it confers resistance to clinically available aminoglycosides, including the novel plazomicin.
