Honest and difficult discussions between the team and the patient are necessary throughout the entire care process.

Headache characteristics and postoperative course in Chiari I malformation

Summary: This study retrospectively analyzed headache characteristics and surgical outcomes in 65 patients with Chiari I malformation. The results showed that frequency, intensity, and use of analgesics significantly decreased in patients with cough headache after decompression surgery, while improvement in atypical headache was less pronounced. Furthermore, the study suggested a positive correlation between the degree of preoperative tonsillar descent and postoperative headache relief.

Programming of cardiometabolic health: the role of maternal and fetal hyperinsulinaemia

Summary: Obesity and gestational diabetes during pregnancy can have both short-term and long-term consequences for both mother and child. One common feature of these conditions is maternal hyperinsulinaemia, which can affect placental growth, angiogenesis, and lipid metabolism. The fetus is exposed to high levels of glucose due to maternal hyperglycaemia, leading to fetal hyperglycaemia/hyperinsulinaemia and complications in fetal development. These changes may increase the risk of cardiometabolic diseases in the offspring later in life. This review summarizes the impact of maternal and fetal hyperinsulinaemia on various aspects and discusses interventions to alleviate these effects.

However, patients are usually not diagnosed with CM if MRI shows less than 3-5 mm of TH and no syrinx. Recent MRI morphometric analysis has detected anatomical abnormalities that explain these symptoms.
