Development and characterization of cell models harbouring mtDNA deletions for i n vitro study of Pearson syndrome

Summary: Pearson syndrome is a rare multisystem disease characterized by refractory sideroblastic anemia caused by single large-scale mitochondrial DNA deletions (SLSMDs). In this study, three different cell models carrying an SLMSD were investigated, and all exhibited abnormal mitochondrial ultrastructure and defective oxidative phosphorylation system function. Interestingly, induced pluripotent stem cells were able to differentiate into three germ layers, while cybrid clones showed mitochondrial dysfunction only at a high heteroplasmy level.

Pearson syndrome: a multisystem mitochondrial disease with bone marrow failure

Summary: Pearson syndrome (PS) is a rare and deadly mitochondrial disorder characterized by anemia in infancy. Bone marrow cytology and detection of SLSMDs are key in diagnosing PS. Non-hematological symptoms are often absent at initial presentation, making PS an important differential diagnosis for isolated hypogenerative anemia in infants. While two-thirds of patients experience spontaneous resolution of anemia between the ages of 1 to 3, they may develop multisystem complications throughout the course of the disease. Currently, there is no curative therapy for PS, highlighting the need for the development of new treatment options.

Modulating mitochondrial DNA mutations: factors shaping heteroplasmy in the germ line and somatic cells

Summary: It was previously believed that humans only have one type of mitochondrial DNA (mtDNA), but recent studies have discovered that mixed populations of mtDNA are actually common. The levels of heteroplasmy, or mixed mtDNA, can change significantly during transmission and contribute to mitochondrial diseases and other late onset disorders. While initially thought to be stochastic, the segregation of mtDNA heteroplasmy is now known to be regulated by specific mechanisms. This review provides a synthesis of recent literature on the selection mechanisms for and against mtDNA variants and highlights the gaps in our understanding of this process.

Varied Responses to a High m.3243A>G Mutation Load and Respiratory Chain Dysfunction in Patient-Derived Cardiomyocytes

Summary: The m.3243A>G mutation in mitochondrial tRNA-Leu(UUR) is a common pathogenic mutation in humans, but the causes for the clinical variability are unknown. In this study, iPSC-CMs derived from two different m.3243A>G patients were used to investigate the molecular effects of high mutation load on cardiomyocyte functionality. The results showed that high mutation load impaired mitochondrial respiration in both patients' cells, but the downstream consequences varied, suggesting the influence of other factors such as glucose metabolism, ATP levels, and intracellular calcium signaling.

A small-molecule compound called tryptolinamide (TLAM) was discovered to activate mitochondrial respiration and rescue defects in neuronal differentiation in iPSCs with a high ratio of mutant mtDNA by inhibiting PFK1 and promoting AMPK-mediated fatty-acid oxidation. TLAM redirects carbon flow from glycolysis to the pentose phosphate pathway to enhance anti-oxidative potential. These findings suggest that PFK1 could be a potential therapeutic target for mitochondrial diseases.
