Acute Kidney Injury and Gut Dysbiosis: A Narrative Review Focus on Pathophysiology and Treatment

Summary: Acute kidney injury (AKI) and gut dysbiosis have a bidirectional relationship, but the mechanisms behind this interaction are not clear. This review focuses on the pathophysiology of this bidirectional interplay and discusses AKI treatment based on it. Macrophages, neutrophils, and T helper type 17 cells play crucial roles in AKI-induced gut dysbiosis. Dysbiosis-induced overproduction of gut-derived uremic toxins and insufficient generation of short-chain fatty acids are the main factors worsening AKI. Improving the gut microbiome to break this vicious cycle is a potential treatment for AKI.

Predictive factors of postoperative paralytic ileus following abdominal surgery: a clinical study

Summary: This retrospective study assessed the risk factors, diagnostic techniques, and management strategies for post-operative ileus. The study found that high BMI, cardiac diseases, previous abdominal surgery, chemotherapy, low preoperative albumin levels, gastrointestinal surgeries, postoperative hypokalemia, and prolonged hospital stay were independent risk factors for post-operative ileus.

Medical Management of Constipation in Elderly Patients: Systematic Review

Summary: This study reviewed the latest literature on the effect and safety of laxatives in elderly patients with constipation, finding that bulk laxatives, osmotic laxatives, and stimulant laxatives with or without fiber were more effective than placebo in short-term treatment. The frequency and severity of adverse effects of laxatives were similar between study groups. Although the quality of included studies was not high, polyethylene glycol seems to be a safe and effective option for long-term use in elderly patients.

Risk factors for upper and lower type prolonged postoperative ileus following surgery for Crohn's disease

Summary: This study identified different risk factors for upper and lower PPOI after ileocecal resection in patients with CD. A differentiated upper/lower type approach should be considered in future research and clinical practice. High-risk patients for each type of PPOI should be closely monitored, and modifiable risk factors, such as preoperative anemia and opioids, should be avoided if possible.

Gastrointestinal side effects associated with antidepressant treatments in patients with major depressive disorder: A systematic review and meta-analysis

Summary: This meta-analysis investigated the short-term rates of gastrointestinal side effects in MDD patients treated with second-generation antidepressants. The results showed that all considered antidepressants had higher rates of gastrointestinal side effects compared to placebo, with escitalopram and sertraline being the least tolerated, while mirtazapine had fewer side effects.
