Effectiveness of a culturally competent care intervention in reducing disparities in Hispanic live donor kidney transplantation: A hybrid trial

Summary: The study found that the Northwestern Medicine Hispanic Kidney Transplant Program successfully increased the likelihood of recipients receiving living donor kidney transplants, particularly at the intervention site with higher fidelity. For Hispanic recipients at intervention site A, the likelihood of receiving LDKTs significantly increased after the intervention, while there was no significant change in the paired control sites.

Summary: The National Kidney Foundation and the American Society of Nephrology have made recommendations regarding the consideration of race in GFR estimation. They suggest implementing the CKD-EPI creatinine equation without the race variable in all US laboratories and promoting the use of cystatin C for more accurate estimation. They also encourage research on new filtration markers and interventions to eliminate racial disparities. The unified approach should be adopted nationwide.

Financial Feasibility Analysis of a Culturally and Linguistically Competent Hispanic Kidney Transplant Program

Summary: This study assessed the financial feasibility of implementing a culturally targeted Hispanic Kidney Transplant Program (HKTP) at two other transplant centers, finding that the staffing costs involved were very low compared to the total costs. The results suggest that implementing HKTP at other transplant centers may present a potential business case for reducing disparities in live donor kidney transplantation among Hispanic/Latino populations.

Living Kidney Donors' Financial Expenses and Mental Health

Summary: Living kidney donors who incur higher expenses postdonation tend to have significantly worse mental health-related quality of life, with a trend towards increased anxiety and depression three months after donation. However, these differences are no longer significant 12 months after donation. Efforts should be made to minimize donor-incurred expenses and provide adequate psychosocial support to donors.

Association of the Estimated Glomerular Filtration Rate With vs Without a Coefficient for Race With Time to Eligibility for Kidney Transplant

Summary: In a cohort study of 1658 self-identified Black adults with chronic kidney disease, commonly used estimates of kidney function did not correspond well with directly measured kidney function. Estimating kidney function not including a coefficient for race (vs including a race coefficient) was significantly associated with a shorter time to achieving an estimated glomerular filtration rate less than 20 mL/min/1.73 m(2), a key threshold of kidney function for referral and listing for kidney transplant. The findings suggest that biases in race-based glomerular filtration rate estimates may be associated with delays in potential kidney transplant eligibility.

Transplant outcomes using kidneys from high KDPI acute kidney injury donors

Summary: This study retrospectively assessed kidney transplant outcomes from high kidney donor profile index (KDPI >= 85%) donors with acute kidney injury (AKI). The findings showed that higher levels of AKI were associated with increased rates of delayed graft function and one-year death-censored graft failure.

Wages, Travel, and Lodging Reimbursement by the National Kidney Registry: An Important Step Toward Financial Neutrality for Living Kidney Donors in the United States

Summary: The study found that the Donor Shield program provided important financial support to US donors between January 2019 and February 2020, with 326 donors receiving reimbursements totaling $647,384.45, mostly covering lost wages. Survey results showed that 81% of respondents reported that the lack of these reimbursements would have posed a financial hardship, and 4% said they would have been unable to donate without this support.
