Idiopathic pulmonary fibrosis: state of the art for 2023

Summary: Idiopathic pulmonary fibrosis (IPF) is a progressive fibrotic lung disease with worsening respiratory symptoms and physiological impairment. The prevalence of IPF has been increasing due to improved awareness, increased use of computed tomography scans, and other factors. There have been significant advances in understanding the pathobiology of IPF and identifying genetic and nongenetic contributors. The disease course and progression of IPF are unpredictable, and treatment with antifibrotic therapies can slow down decline of lung function.

It is classified as a primary antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) and hypereosinophilic syndrome (HES), causing variable clinical presentation. The disease is challenging to diagnose, and there is a need for uniform diagnostic criteria and biomarkers. Despite progress in understanding the disease, there is still reliance on corticosteroid therapy, highlighting the need for more effective treatment options.

However, the lack of equol-producing bacteria in the gastrointestinal tract of over half of the human population complicates the interpretation of data collected from humans. Rodent models, which efficiently produce equol, also pose challenges in translating findings to humans. The development of model systems to manipulate equol production and control confounding factors is needed to determine the therapeutic approaches for maximizing equol production and improving the health of postmenopausal women consuming soy.
