Non-invasive stereotactic arrhythmia radiotherapy for ventricular tachycardia: results of the prospective STARNL-1 trial

Martijn H. van der Ree et al.

Summary: This study aims to evaluate the efficacy and safety of Stereotactic Arrhythmia Radiotherapy (STAR). It found that STAR can reduce the number of treated ventricular tachycardia episodes by more than 50% in 67% of patients with therapy-refractory VT. No decrease in cardiac and pulmonary function or treatment-related serious adverse events were observed during follow-up.

STereotactic Arrhythmia Radioablation (STAR): the Standardized Treatment and Outcome Platform for Stereotactic Therapy Of Re-entrant tachycardia by a Multidisciplinary consortium (STOPSTORM.eu) and review of current patterns of STAR practice in Europe

Melanie Grehn et al.

Summary: The STOPSTORM consortium, funded by the EU Horizon 2020 Framework, aims to establish a standardized treatment and outcome platform for investigating STAR in ventricular tachycardia. The consortium consists of 31 clinical and research institutions and is divided into nine work packages. A comprehensive questionnaire was conducted to review the current clinical practice of STAR in Europe, highlighting potential areas of optimization and harmonization.

The effect of ionizing radiation through cardiac stereotactic body radiation therapy on myocardial tissue for refractory ventricular arrhythmias: A review

John Whitaker et al.

Summary: Cardiac stereotactic body radiation therapy (cSBRT) is a non-invasive treatment modality that has been effective for ventricular arrhythmias. Despite some success, there is limited understanding of the radiobiology of its anti-arrhythmic effect and unclear mechanisms. Future research is needed to optimize the treatment's efficacy and explore its potential.

Substrate Modification Using Stereotactic Radioablation to Treat Refractory Ventricular Tachycardia in Patients With Ischemic Cardiomyopathy

Pierre C. Qian et al.

Summary: The study shows the feasibility of using radioablation for arrhythmogenic substrate modification in patients with ischemic cardiomyopathy, but the reduction in device shocks and VT burden was suboptimal, with a significant mortality rate.

Stereotactic radioablation for the treatment of ventricular tachycardia: preliminary data and insights from the STRA-MI-VT phase Ib/II study

Corrado Carbucicchio et al.

Summary: The preliminary findings of the STRA-MI-VT Study suggest that SBRT could be a potential alternative option for treating VT in patients with structural heart disease, as it resulted in a significant reduction in VT episodes over a 6-month period without any serious treatment-related adverse events. Further clinical investigation is warranted to confirm the safety and efficacy of SBRT in this patient population.

Early Changes in Rat Heart After High-Dose Irradiation: Implications for Antiarrhythmic Effects of Cardiac Radioablation

Myung-Jin Cha et al.

Summary: High-dose cardiac radioablation induced mononuclear cell-related intracellular and extracellular edema, diffuse vacuolization, and widened intercalated discs in rat hearts within 1 month. ECG showed intracardiac conduction delay and significant ST-segment changes. These findings suggest that early antiarrhythmic effects following cardiac radioablation are due to conduction disturbances and membrane potential alterations, rather than necrosis.

Recommendations regarding cardiac stereotactic body radiotherapy for treatment refractory ventricular tachycardia

David Krug et al.

Summary: Experts established a consensus on the use of cardiac stereotactic body radiotherapy (CSBRT) for refractory ventricular tachycardia (VT). The procedure is considered experimental and should be conducted in specialized centers with expertise in managing ventricular arrhythmias and stereotactic body radiotherapy for moving targets. Patient selection is crucial and interdisciplinary collaboration is needed for optimal results.
