Perioperative clinical practice in liver transplantation: a cross-sectional survey

Francois M. Carrier et al.

Summary: The objective of this study was to describe some components of perioperative care in liver transplantation as reported by clinicians. A cross-sectional clinical practice survey was conducted, and the results showed significant equipoise on the optimal perioperative management of this population.

Preoperative Fibrinogen Level and Bleeding in Liver Transplantation for End-stage Liver Disease: A Cohort Study

Francois Martin Carrier et al.

Summary: This study found that a lower preoperative fibrinogen level is associated with increased bleeding, blood transfusions, and estimated perioperative blood loss in liver transplantation. However, there was no association between preoperative fibrinogen level and reinterventions, 1-year graft survival, or 1-year patient survival.

Consensus Statement on Hemostatic Management, Anticoagulation, and Antiplatelet Therapy in Liver Transplantation

Eva Montalva et al.

Summary: The increasing use of anticoagulation and antiplatelet therapies in liver transplant patients presents a challenge for transplant teams. The lack of scientific evidence makes it difficult to monitor their effectiveness and manage complications. The Spanish Society of Liver Transplantation and the Spanish Society of Thrombosis and Haemostasis aimed to address this issue and provide guidance through a consensus statement.

Intraoperative transfusion management, antifibrinolytic therapy, coagulation monitoring and the impact on short-term outcomes after liver transplantation-A systematic review of the literature and expert panel recommendations

Uzung Yoon et al.

Summary: This systematic review evaluates the impact of intraoperative transfusion and coagulation management strategies on immediate and short-term outcomes after liver transplantation. The findings suggest that specific blood product transfusion practices are not recommended, while cell salvage and educational interventions can significantly reduce blood product transfusion. Routine use of antifibrinolytics is not recommended, while the use of viscoelastic testing is recommended.

Association between red blood cell distribution width and blood transfusion in patients undergoing living donor liver transplantation: Propensity score analysis

Ji-Hoon Sim et al.

Summary: This study investigated the association between red blood cell distribution width (RDW) and intraoperative transfusion in living donor liver transplantation (LDLT) recipients. The results showed that preoperative RDW level was significantly associated with intraoperative transfusion, and RDW had the predictive power for intraoperative transfusion.

Summary: The study confirms that preoperative fibrinogen and hemoglobin levels are indicators of potential massive perioperative blood loss during orthotopic liver transplantation, which has significant implications for preoperative preparation and patient management.

Techniques for blood loss estimation in major non-cardiac surgery: a systematic review and meta-analysis

Alexandre Tran et al.

Summary: Estimated blood loss (EBL) is a crucial tool for clinical decision-making and surgical outcomes research. In this systematic review of techniques for intraoperative blood loss estimation in major non-cardiac surgery, visual estimation was found to be the most studied and tended to provide lower EBL values compared to other methods. However, there is significant case-by-case variation and disagreement between techniques, suggesting the need for a universal, practical, and reproducible method for blood loss evaluation.

Fibrinogen in traumatic haemorrhage

James Winearls et al.

Summary: Recent advances in understanding the pathophysiological processes related to traumatic hemorrhage and trauma-induced coagulopathy have improved outcomes for seriously injured trauma patients, but a significant number still die from hemorrhage. Fibrinogen plays a crucial role in hemostasis, and early replacement in severely injured trauma patients may improve outcomes, although evidence is limited.
