Molecular Mechanisms and Physiological Changes behind Benign Tracheal and Subglottic Stenosis in Adults

Alessandro Marchioni et al.

Summary: Laryngotracheal stenosis (LTS) is a complex and heterogeneous disease with unclear pathogenesis. Recent research suggests that LTS is a fibroinflammatory event caused by immunological alterations. Factors such as growth factors, cytokines, altered fibroblast function, and genetic susceptibility can interact to result in aberrant and fibrotic wound healing. Additionally, physiological derangement caused by LTS may play a role in promoting dysregulated response to laryngo-tracheal mucosal injury.

An Updated Review of Subglottic Stenosis: Etiology, Evaluation, and Management

Luke J. Pasick et al.

Summary: The purpose of this review is to summarize the recent literature on subglottic stenosis (SGS), including its etiology, diagnosis, and treatment. Recent findings suggest that inflammation plays a significant role in the development of both iatrogenic and idiopathic SGS. The PD-1/PD-L1 axis may be a potential therapeutic target for inhibiting SGS. Surgical management studies show that endoscopic approaches have a higher recurrence rate compared to open techniques. In-office injections are gaining popularity and show promise in treating SGS.

Long-term intubation and high rate of tracheostomy in COVID-19 patients might determine an unprecedented increase of airway stenoses: a call to action from the European Laryngological Society

Cesare Piazza et al.

Summary: This paper compiles the opinions of experts in the field of Laryngotracheal Stenosis Committee of the European Laryngological Society, aiming to raise awareness within the medical community about the potential increase in cases of COVID-19-related laryngotracheal stenosis (LTS), and advocating for a more rational concentration of these cases in specialist airway centers.
