The effect of group-based cognitive behavioural therapy for insomnia in patients with rheumatoid arthritis: a randomized controlled trial

Summary: The objective of the study was to compare the effects of cognitive behavioural therapy for insomnia (CBT-I) to usual care on sleep efficiency. The study found that CBT-I did not significantly improve sleep efficiency measured by polysomnography (PSG). However, CBT-I showed significant improvement in patient-reported sleep and RA-related outcomes.

Touch neurons underlying dopaminergic pleasurable touch and sexual receptivity

Summary: By studying mice, it is found that sensory neurons labeled by Mrgprb4 are able to detect mechanical stimulation and play a crucial role in sexual receptivity and dopamine release. Optogenetic stimulation of these neurons can induce a conditioned place preference and a copulatory posture even in social isolation. In the absence of Mrgprb4-lineage neurons, female mice no longer find male mounts rewarding and show aggression instead.

Caudovirales bacteriophages are associated with improved executive function and memory in flies, mice, and humans

Summary: Growing evidence suggests a connection between the gut microbiome and cognition, with viruses as a commonly overlooked component of the gut virome. This study found that higher levels of Caudovirales and Siphoviridae in the gut microbiome were associated with better executive processes and verbal memory, while increased Microviridae levels were linked to greater impairment in executive abilities. Transplanting microbiota with specific high levels of Caudovirales resulted in improved memory performance in mice and up-regulated memory-promoting genes in the prefrontal cortex. Adding lactococcal Siphoviridae phages to the diet of fruit flies also enhanced memory scores and upregulated memory-related brain genes. These findings suggest that bacteriophages play a role in the microbiome-brain axis.

Remotely delivered cognitive behavioural and personalised exercise interventions for fatigue severity and impact in inflammatory rheumatic diseases (LIFT): a multicentre, randomised, controlled, open-label, parallel-group trial

Summary: Background: Chronic fatigue is a poorly managed problem in people with inflammatory rheumatic diseases. Cognitive behavioural approaches (CBA) and personalised exercise programmes (PEP) can be effective, but they are not often implemented because their effectivenesses across the different inflammatory rheumatic diseases are unknown and regular face-to-face sessions are often undesirable, especially during a pandemic. We hypothesised that remotely delivered CBA and PEP would effectively alleviate fatigue severity and life impact across inflammatory rheumatic diseases. Methods: LIFT is a multicentre, randomised, controlled, open-label, parallel-group trial to assess usual care alongside telephone-delivered CBA or PEP against usual care alone in UK hospitals. Patients with any stable inflammatory rheumatic disease were eligible if they reported clinically significant, persistent fatigue. Treatment allocation was assigned by a web-based randomisation system. CBA and PEP sessions were delivered over 6 months by trained health professionals in rheumatology. Coprimary outcomes were fatigue severity (Chalder Fatigue Scale) and impact (Fatigue Severity Scale) at 56 weeks. The primary analysis was by full analysis set. This study was registered at ClinicalTrials.gov (NCT03248518). Findings: From Sept 4,2017, to Sept 30,2019, we randomly assigned and treated 367 participants to PEP (n=124; one participant withdrew after being randomly assinged), CBA (n=121), or usual care alone (n=122), of whom 274 (75%) were women and 92 (25%) were men with an overall mean age of 57.5 (SD 12.7) years. Analyses for Chalder Fatigue Scale induded 101 participants in the PEP group, 107 in the CBA group, and 107 in the usual care group and for Fatigue Severity Scale included 101 in PEP, 106 in CBA, and 107 in usual care groups. PEP and CBA significantly improved fatigue severity (Chalder Fatigue Scale; PEP: adjusted mean difference -3.03 [97.5% CI -5.05 to -1.02], p=0.0007; CBA: -2.36 [-4.28 to -0.44], p=0.0058) and fatigue impact (Fatigue Severity Scale; PEP: -0.64 [-0.95 to -0.33], p<0.0001; CBA: -0.58 [-0.87 to -0.28], 0.0001); compared with usual care alone at 56 weeks. No trial-related serious adverse events were reported. Interpretation: Telephone-delivered CBA and PEP produced and maintained statistically and clinically significant reductions in the severity and impact of fatigue in a variety of inflammatory rheumatic diseases. These interventions should be considered as a key component of inflammatory rheumatic disease management in routine clinical practice. Copyright (C) 2022 The Author(s). Published by Elsevier Ltd.

Comparison of the effectiveness of pilates exercises, aerobic exercises, and pilates with aerobic exercises in patients with rheumatoid arthritis

There was no significant difference in outcomes among the three treatment groups.
