A metagenomics method for the quantitative detection of bacterial pathogens causing hospital-associated and ventilator-associated pneumonia

Summary: The management of ventilator-associated and hospital-acquired pneumonia requires rapid and accurate quantitative detection of the infecting pathogen(s). We propose a metagenomics next-generation sequencing (mNGS) assay with an internal sample processing control (SPC) for this purpose. The assay showed high sensitivity compared to microbial culture, but also had some false-positive results.

Mycobacterium chelonae Infection Identified by Metagenomic Next-Generation Sequencing as the Probable Cause of Acute Contained Rupture of a Biological Composite Graft-A Case Report

Summary: This case report presents a 72-year-old female patient who experienced acute rupture of a biological composite graft 21 months after aortic valve and ascending aorta replacement due to aortic dissection. Although acid-fast bacilli were detected in intraoperative biopsies using Auramine-rhodamine staining, classical culture and molecular methods failed to identify any organism. Metagenomic analysis revealed an infection with Mycobacterium chelonae, which was confirmed by target-specific qPCR. The study highlights the importance of multiple testing to enhance pathogen identification in complex samples.

Summary: mNGS has shown superior performance in the diagnosis of suspected pulmonary infection, especially in detecting Mycobacterium tuberculosis, atypical pathogens, viruses, and fungi. It significantly improves the diagnosis and guides targeted treatment and clinical management. The unbiased nature of mNGS makes it a promising microbiological detection and diagnostic method in clinical practice.

Metagenomic next-generation sequencing for accurate diagnosis and management of lower respiratory tract infections

Summary: This study retrospectively reviewed the clinical data of patients with suspected lower respiratory tract infections at Wuhan Union Hospital. The results showed that mNGS had a high sensitivity and shorter processing time compared to culture test. Antibiotic treatment could be adjusted or confirmed based on the mNGS results, leading to significant improvement in clinical symptoms and inflammatory indicators for patients.

Summary: The study evaluated the performance of FA-PP in detecting bacterial pathogens and antibiotic resistance genes in respiratory specimens, showing that it performed well in detecting pathogens in most specimens. The positive and negative percentage agreement values of FA-PP compared to culture were high, but there were some false-negative results. In addition, there was some discordance between the semi-quantification results of FA-PP and culture results.
