Does frailty status predict outcome in major trauma in older people? A systematic review and meta-analysis

Summary: Frailty has a significant impact on the outcomes of major trauma in older people, and it may be a stronger predictor than age. This systematic review provides evidence that frailty is associated with increased in-hospital mortality, longer hospital stays, adverse discharge disposition, and in-hospital complications in older trauma patients.

Association of Trauma Molecular Endotypes With Differential Response to Transfusion Resuscitation Strategies

Summary: This study found that plasma biomarkers at hospital arrival can be used to identify different molecular endotypes in trauma patients and assess their treatment response to 1:1:1 and 1:1:2 resuscitation strategies. The results showed that different endotypes were associated with differential treatment response and 30-day mortality in trauma patients. Therefore, personalized treatment strategies may be more effective for patients at high risk for adverse outcomes.

Summary: The study revealed that HRGEC injury and SDC-1 shedding contribute to increased permeability and proteinuria formation in lupus nephritis. Inhibiting extracellular HMGB1 and/or downstream TLR4/MyD88/NF-kappa B/p65 signaling pathway also improved GEC monolayer permeability, reduced glycocalyx shedding, enhanced intercellular tight junctions and cytoskeletal arrangement, and decreased NO level and VCAM-1 expression, indicating the potential therapeutic target for lupus nephritis treatment.
