Clinical Pharmacogenetics Implementation Consortium Guideline for CYP2C19 Genotype and Clopidogrel Therapy: 2022 Update

Craig R. Lee et al.

Summary: CYP2C19 enzyme plays a role in the activation of clopidogrel, and the genotype of CYP2C19 affects the formation of active metabolites of clopidogrel. Thus, using CYP2C19 genotype to guide the use of clopidogrel can reduce the risk of reduced platelet inhibition and major adverse cardiovascular and cerebrovascular events, particularly for intermediate metabolizers.

The Cytokine Release Syndrome and/or the Proinflammatory Cytokines as Underlying Mechanisms of Downregulation of Drug Metabolism and Drug Transport: A Systematic Review of the Clinical Pharmacokinetics of Victim Drugs of this Drug-Disease Interaction Under Different Clinical Conditions

Milo Gatti et al.

Summary: This systematic review analyzed the impact of cytokine modulation on the pharmacokinetics of victim drugs. The results showed that cytokine modulation may cause downregulation of metabolism/transport of victim drugs, which could have significant clinical consequences.

Impact of Acute Inflammation on Cytochromes P450 Activity Assessed by the Geneva Cocktail

Camille Lenoir et al.

Summary: The study aimed to evaluate the impact of orthopedic surgery on the activity of human CYP enzymes, finding that acute inflammation significantly decreased the activity of CYP1A2, CYP2C19, and CYP3A, while increasing the activity of CYP2B6 and CYP2C9.

Modeling Approach to Predict the Impact of Inflammation on the Pharmacokinetics of CYP2C19 and CYP3A4 Substrates

Florian Simon et al.

Summary: The study aimed to quantify the impact of inflammation on CYP2C19 and CYP3A4 activities and predict the pharmacokinetic profile of their substrates based on CRP concentration. Combining in vitro experiments and clinical data, physiologically based pharmacokinetic models were established, and the results indicated that the pharmacokinetic changes induced by inflammation could be accurately captured by the models.

The management of acute coronary syndromes in patients presenting without persistent ST-segment elevation: key points from the ESC 2020 Clinical Practice Guidelines for the general and emergency physician

Ramesh Nadarajah et al.

Summary: This article provides an overview of the significant advances in the diagnosis and management of non-ST-segment elevation myocardial infarction, as reflected in the 2020 European Society of Cardiology Clinical Practice Guidelines. It focuses on recommendations and evidence basis in three key areas: diagnosis, pathways, and treatment, while highlighting gaps in the evidence base, such as optimal management for older people and antiplatelet regimes in cases of delayed angiography.

CYP2C19 genotype has prognostic value in specific populations following coronary stenting

Wenyao Wang et al.

Summary: This study found that post-PCI patients with the CYP2C19 poor metabolizer genotype had the highest rate of major adverse cardio- and cerebro-vascular events (MACCE), while those with the extensive metabolizer genotype had the lowest rate. There was no significant difference in major bleeding rates between the three groups. The prognostic value of the CYP2C19 genotype was observed in male patients, those over 60 years old, with a BMI over 24 kg/m(2), SYNTAX score over 15, current smokers, and patients without chronic kidney disease.
