Forecasting individual progression trajectories in Alzheimer's disease

Etienne Maheux et al.

Summary: This study developed a statistical model, AD Course Map, for predicting the progression of Alzheimer's disease (AD) based on current medical and radiological data. The model was tested on a large dataset of over 96,000 cases and showed high accuracy in predicting clinical endpoints. By enriching the population with predicted progressors, the required sample size for trials could be reduced by 38% to 50%.

Forecasting individual progression trajectories in Huntington disease enables more powered clinical trials

Igor Koval et al.

Summary: This study utilizes disease course mapping to forecast biomarker progression for individual carriers of the pathological CAG repeat expansions responsible for Huntington disease, in order to select participants at risk for progression and compute the power of trials for such an enriched population, ultimately reducing sample sizes and ensuring a more homogeneous group of participants.

Parkinson Disease Propagation Using MRI Biomarkers and Partial Least Squares Path Modeling

Nadya Pyatigorskaya et al.

Summary: Using multimodal MRI and PLS-PM, this study found that PD patients with RBD followed the Braak brainstem-to-cortex model of neurodegeneration, while those without RBD followed the cortex-to-brainstem model.

Sequence of clinical and neurodegeneration events in Parkinson's disease progression

Neil P. Oxtoby et al.

Summary: This study estimated the sequence of Parkinson's disease progression in patients at elevated risk for dementia, beginning with classic prodromal features, offering a route to earlier and more precise detection and characterizing variability in the sequence.

Prodromal Parkinson disease subtypes - key to understanding heterogeneity

Daniela Berg et al.

Summary: Parkinson's disease begins its pathological processes and neurodegeneration long before clinical diagnosis, allowing for identification of individuals likely to develop the disease through risk and prodromal markers. Heterogeneity in clinical and prodromal PD suggests that subtypes exist, differentiated by clinical manifestations, pathomechanisms, and progression patterns. Understanding this variability is crucial for early and accurate diagnosis, as well as for targeting neuroprotective interventions effectively.

AD Course Map charts Alzheimer's disease progression

Igor Koval et al.

Summary: This paper introduces a spatiotemporal atlas called AD Course Map for Alzheimer's disease progression, highlighting strong genetic determinants for the progression and predicting patients' cognitive decline with better accuracy than other benchmarked methods. The map also offers new tools for exploring the progression of Alzheimer's disease and personalizing patients care through simulations of virtual patient cohorts.

Longitudinal Changes in Parkinson's Disease Symptoms with and Without Rapid Eye Movement Sleep Behavior Disorder: The Oxford Discovery Cohort Study

Yaping Liu et al.

Summary: Patients with PD comorbid with rapid eye movement sleep behavior disorder (RBD) exhibit more severe motor and cognitive symptoms, progress faster, and are associated with an increased risk for mild cognitive impairment, freezing of gait, and frequent falling. This suggests the presence of a more aggressive subtype of PD that can be identified early and has significant clinical implications.

Discovery of Parkinson's disease states and disease progression modelling: a longitudinal data study using machine learning

Kristen A. Severson et al.

Summary: The study developed a statistical progression model of early Parkinson's disease that accounts for intra-individual and inter-individual variability and medication effects. The predictive model discovered non-sequential, overlapping disease progression trajectories, supporting the use of non-deterministic disease progression models, and suggesting static subtype assignment might be ineffective at capturing the full spectrum of Parkinson's disease progression.
