The association between cancer diagnosis, care, and outcomes in 1 million patients hospitalized for acute pulmonary embolism

Aaron Shengting Mai et al.

Summary: This study evaluated the clinical care provided to cancer patients hospitalized for acute pulmonary embolism and assessed the association between type of cancer, in-hospital care, and clinical outcomes. The results showed that cancer patients were less likely to receive invasive management compared to non-cancer patients. Cancer patients had higher odds of mortality, with lung cancer patients having the highest mortality and hemorrhagic stroke risk, while colorectal cancer patients had the highest bleeding risk.

In-hospital prognosis of malignancy-related pulmonary embolism: an analysis of the national inpatient sample 2016-2018

Dae Yong Park et al.

Summary: A substantial proportion of patients with malignancy develop pulmonary embolism (PE), which significantly worsens the prognosis and ranks as one of the leading causes of mortality in these patients. This retrospective study examined the prognosis of PE in 17 different types of malignancies and found that PE in all types of malignancies had significantly higher odds of in-hospital mortality compared to PE without malignancy. Esophageal cancer had the highest odds of in-hospital mortality, followed by gastric, ovarian, and lung cancer.

Catheter-directed interventions for pulmonary embolism

Ajar Kochar et al.

Summary: Pulmonary embolism (PE) is a common, life-threatening and recurrent condition. The management of acute PE requires early detection, risk stratification and multidisciplinary decision-making. Catheter-directed interventions can help improve the condition of patients by increasing the cross-sectional area of the pulmonary vasculature, but integrating clinical trial evidence into individualized care remains a challenge.

In-hospital outcomes of catheter-directed thrombolysis in patients with pulmonary embolism

Lukas Hobohm et al.

Summary: Catheter-directed thrombolysis (CDT) showed lower in-hospital mortality rates compared to systemic thrombolysis in patients with acute pulmonary embolism (PE) in a German nationwide inpatient cohort, but the risk of intracranial bleeding in patients receiving CDT was not negligible. Prospective controlled data are needed to further evaluate the true value of CDT in acute PE treatment.

Percutaneous coronary intervention in patients with cancer and readmissions within 90 days for acutemyocardial infarction and bleeding in the USA

Chun Shing Kwok et al.

Summary: This study evaluated the rates of readmissions within 90 days for AMI and bleeding among patients with cancer who undergo PCI. The results showed that patients with cancer, especially those with active cancer, have increased risk for readmissions for AMI or bleeding following PCI. The magnitude of risk depends on both cancer type and the presence of metastasis.

Pulmonary embolism at autopsy in cancer patients

Inge A. Gimbel et al.

Summary: The study found that 12.4% of cancer patients had pulmonary embolism at autopsy, with 90.2% being thrombotic embolism. Venous thromboembolism occurred in 12.8% of the patients.

Antithrombotic Therapy for VTE Disease Second Update of the CHEST Guideline and Expert Panel Report

Scott M. Stevens et al.

Summary: This is the 2nd update to the 9th edition of guidelines, providing recommendations on 17 PICO questions, with four new questions addressed. Strong and weak recommendations were generated based on high-, moderate-, and low-certainty evidence, resulting in 29 guidance statements, 13 of which are strong recommendations. New evidence has emerged since 2016, improving the standard of care for VTE patients, but substantial uncertainty remains regarding certain important management questions.

Summary: This is the 2nd update to the 9th edition of the guidelines, with 29 guidance statements generated using GRADE methodology. These statements cover various aspects of antithrombotic management of VTE, from initial management through secondary prevention and risk reduction of postthrombotic syndrome.
