Reverse Triggering during Controlled Ventilation From Physiology to Clinical Management

Antenor Rodrigues et al.

Summary: Reverse triggering is a common phenomenon in sedated critically ill patients under controlled ventilation, occurring in 30-55% of cases. It can be caused by entrainment of the patient's intrinsic respiratory rhythm or local reflexes. Diagnosing reverse triggering can be challenging, but can be done through ventilation waveforms or more sophisticated methods. Managing reverse triggering is not well-established, but evidence-based management can be guided by physiological principles.

Summary: Survivors of acute respiratory distress syndrome (ARDS) may experience reduced health-related quality of life (HRQoL) and impairments in physical and mental health following ICU discharge, which may not fully recover up to 5 years.

Summary: Real-world data from 2010 to 2019 indicate that patients with ARDS in South Korea have experienced changes in mortality, treatment, and healthcare costs. Despite the increased financial burden, mortality among patients with ARDS has decreased due to advances in disease management.

Summary: The use of early, continuous-infusion NMBAs in subjects with ARDS reduces short-term mortality, improves oxygenation, and decreases the risk of ventilator-induced lung injury, but does not significantly affect long-term mortality.

Prolonged Hospitalization Following Acute Respiratory Failure

Meghan Marmor et al.

Summary: The study found that from 2004 to 2014, the incidence and mortality of patients with acute respiratory failure and prolonged hospitalization decreased, while the cost per patient increased. However, the total cost of care for patients with prolonged hospitalization remained stable, with significant variation in hospital length of stay based on geographic region, hospital teaching status, and patient insurance coverage.
