InCASEOf scoring system for distinction between pachychoroid-associated macular neovascularization and neovascular age-related macular degeneration in patients older than 50 years

Summary: This study developed a novel scoring system to assist in the differential diagnosis between pachychoroid disease-related macular neovascularization (pMNV) and neovascular age-related macular degeneration (AMD) in patients aged 50 years and older. The scoring system, called InCASEOf, was developed based on clinical predictors identified through a comprehensive ophthalmologic evaluation. It showed good accuracy in distinguishing pMNV from neovascular AMD and is accessible to general ophthalmologists.

The establishment of pachychoroid and its spectrum of clinical disorders represents a major conceptual shift, with overlaps in manifestations and treatments making differentiation challenging. Future research is needed to validate the assumptions behind current treatment strategies for pachychoroid neovasculopathy and compare it with other disorders in the pachychoroid spectrum.

Summary: This study in Chinese patients with CSCR found that 7.23% of patients had CNV, with age and duration of CSCR being correlated with CNV occurrence. FIPED was present in all eyes with CNV, and the width and area of PED were greater in the CNV group. Female gender and poor vision were identified as risk factors for CNV in CSCR eyes. Additionally, choroidal thickness was significantly different between eyes with CNV and eyes without CNV.

Summary: This study described the multimodal imaging characteristics of flat irregular pigment epithelial detachment (FIPED) in patients with chronic central serous chorioretinopathy and determined risk factors for vascularized FIPED. Vascularized FIPED was closely associated with Type I choroidal neovascularization and old age was identified as an independent risk factor for vascularized FIPED. Vascularized FIPED was suggested as quiescent choroidal neovascularization, and half-dose photodynamic therapy may be recommended as the first-line therapy in chronic central serous chorioretinopathy complicated with quiescent choroidal neovascularization.
