Comparative Efficacy of Intra-Articular Injection, Physical Therapy, and Combined Treatments on Pain, Function, and Sarcopenia Indices in Knee Osteoarthritis: A Network Meta-Analysis of Randomized Controlled Trials

Summary: This network meta-analysis aimed to identify the comparative efficacy among combined treatments (IAI+PT) in patients with knee osteoarthritis (KOA). The results showed that the combination of DxTP and PT was most effective in reducing pain and restoring global function, while MSC plus PT was most effective in enhancing walking capability. Disease severity was found to moderate the treatment efficacy in terms of pain and walking capability improvement.

The significant symptomatic benefits demonstrate the clinical usefulness of a subsequent trial.

Clinical Effect of Bushen Huoxue Method Combined with Platelet-Rich Plasma in the Treatment of Knee Osteoarthritis and Its Effect on IL-1, IL-6, VEGF, and PGE-2

Summary: This study observed the clinical efficacy of the Bushen Huoxue method combined with platelet-rich plasma (PRP) in the treatment of knee osteoarthritis (KOA) and its impact on serum and joint fluid interleukin-1 (IL-1), interleukin-6 (IL-6), vascular endothelial growth factor (VEGF), and prostaglandin E2 (PGE-2). The results showed that the combination therapy had a higher clinical effectiveness rate compared to PRP alone, with lower osteoarthritis index scores and pain visual analogue scores. Furthermore, the levels of IL-1, IL-6, and PGE-2 in serum and joint fluid were lower in the combination therapy group. Therefore, the combination of PRP and the Bushen Huoxue method can effectively improve the treatment of KOA.

A narrative review of the progress in the treatment of knee osteoarthritis

Summary: This study analyzed the mechanism, benefits, and adverse reactions of commonly used intra-articular injection therapy for knee osteoarthritis (KOA). It was found that corticosteroid injections improved symptoms in the short term, but long-term use may lead to adverse reactions. Intra-articular injection of hyaluronic acid resulted in more adverse reactions. Lack of standardization in platelet-rich plasma (PRP) preparation also led to adverse reactions. Further clinical trials are needed to promote the clinical application of PRP.

PRP injections, particularly with leukocyte-poor formulations, may provide greater therapeutic relief. Combination formulations of HA-PRP conjugates may also have a synergistic effect for clinically significant improvement in pain and function. Further studies on the efficacy of HA-PRP conjugates are warranted.
