Summary: GVHD, both acute and chronic, is a major complication of allogeneic transplantation, and its prevention is crucial for the success of transplants. Current standards revolve around calcineurin inhibitors, but promising translational advances targeting T-cell regulation and co-stimulatory pathways are emerging. These approaches are based on a pathophysiology perspective and show potential for transforming GVHD prophylaxis in the near future.
