Prenatal diagnosis and outcome of fetal urinomas in relation to the underlying etiology

Summary: This study is a retrospective analysis of pregnancies with prenatally diagnosed fetal urinoma in a single center over a 20-year period. The findings showed that lower urinary tract obstruction (LUTO) and ureteropelvic or ureterovesical junction obstruction (UPJO/UVJO) were the main causes of fetal urinoma. Postnatally, loss of function (LOF) in the affected kidney was more common in UPJO/UVJO cases than in LUTO cases. The prognosis for survivors with UPJO/UVJO urinoma is excellent if the contralateral kidney is normal, while the long-term prognosis for LUTO cases depends on postnatal renal and pulmonary function.

Development and in-vitro characterization of a novel fetal vesicoamniotic shunt - The Vortex shunt

Summary: The aim of this study was to develop and test a new vesicoamniotic shunt for the treatment of fetal lower urinary tract obstruction. The results showed that the novel shunt had a higher resistance to dislodgement, easy deployment with ultrasound guidance, and accurate measurement of valve pressure. Further testing in preclinical animal models is currently underway.

Summary: The purpose of this retrospective cohort study was to compare the outcomes of human fetuses with isolated severe lower urinary tract obstructions (LUTO) first treated before 16 weeks of gestation to those treated later in gestation. The study found that early intervention before 16 weeks of gestation may result in a higher rate of normal renal and pulmonary function in surviving fetuses.

Summary: In-utero vesica-amniotic shunting for fetal lower urinary obstruction (LUTO) can improve perinatal survival, but carries significant risks requiring careful patient selection and timing. Improving the accuracy of prenatal diagnosis and identifying potential risk factors for fetal and postnatal renal failure are essential. When selecting the timing for intervention, various factors need to be considered, especially if the goal is renal preservation.
