Lower Airway Dysbiosis Augments Lung Inflammatory Injury in Mild-to-Moderate Chronic Obstructive Pulmonary Disease

Summary: This study investigated whether there is lower airway dysbiosis in mild-to-moderate COPD patients and its possible contributions to COPD pathogenesis. The results showed that the lower airways of COPD patients were enriched with common oral commensals. Additionally, in a preclinical mouse model exposed to cigarette smoke, lower airway dysbiosis with common oral commensals augmented the inflammatory injury, showing similar gene expression characteristics to COPD patients.

Summary: Laboratory animal science associations recommend the use of SPF animals in experiments to achieve accurate results, but their non-natural microbiome may hinder the translation of these results into clinical practice. Inconsistent results in cough response among SPF animals compared to wild-type animals of both sexes led to the suggestion that their usage in basic cough research is questionable. The underlying cause for the altered cough response in SPF animals remains unknown, and it is proposed that it may be microbiome-related, as the relationship between airway reflexes and the airway microbiome has not been studied.

Older individuals, especially those with comorbidities, have the highest death rate. The alteration of gut microbiota, particularly in the elderly, may play an important role in lung homeostasis and the outcome of COVID-19, along with the activation of mast cells and platelets. Therefore, targeted control of intestinal microbiota could be a complementary intervention in the management and prevention of COVID-19.
